Acute Rehabilitation Case Manager
Role Overview
- Acute Rehabilitation Case Manager
- Travel assignment in Tacoma, Washington
- 1 opening
- 8:00 AM to 4:30 PM, Monday through Friday
- 40 hours per week
- No weekend hours
- No call requirements
- 12-week contract
Unit / Setting
- Acute freestanding rehab hospital
- 60-bed inpatient acute rehabilitation hospital
- Patient ratio: 1:15 (+/-)
- Common diagnoses include stroke, multiple medical debilitation, and post-surgical cases
- Average length of stay is about 12 days (+/-)
Required Qualifications
- Valid RN license in Washington
- Associate of Science in Nursing / Associate Degree in Nursing
- Case Management experience within the last year
- 2 years of Case Management experience
- Adult acute hospital experience
- Acute Rehab Experience
- Utilization Review experience
- Utilization Review (InterQual)
Preferred Qualifications
- Case management certification of any type
- Certification in Case Management or Rehabilitation Nursing
- CCM
- ARN certification
- ACM
- ANCC board certification in case management, such as RN-BC
- Additional languages
- Computer skills
- Good, clear communication
- Forward planning
- Ability to work with nursing, therapy, physicians, and families
Additional Notes
- The role supports acute rehabilitation team conferences, and training will be provided for that requirement
- Business casual / hospital professional attire
- Report at 8:00 AM on the first day
