RN Care Manager

Overview

• Location: Chapel Hill, North Carolina
• Setting: Acute care hospital environment
• Schedule: Days, 8 hours per day, 40 hours per week
• Contract length: 13 weeks

Role Summary

Provides ongoing care management support through assessment, planning, implementation, and evaluation of patient needs, with a focus on case management, utilization review, discharge planning, and care coordination for acutely ill patients with complex medical conditions.

Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of Case Management experience
• Case Management experience within the last year
• Experience in an Acute Care setting
• Experience with discharge planning
• Experience with utilization review (InterQual)
• Care Manager experience

Preferred Qualifications

• BSN or MSN from an accredited school of Nursing

Key Responsibilities

• Assess patients and document plans of care
• Prioritize new admissions and review work lists
• Participate in daily care management touchpoints
• Attend and contribute to CAPP meetings and Complex Care Meetings
• Coordinate discharge planning, transition planning, and post-discharge referrals
• Communicate barriers to discharge and collaborate with the multidisciplinary team
• Support family meetings and provide education on community resources
• Monitor observation patients and identify expected discharge dates
• Assist with medication resource management for non-resourced patients
• Refer administrative tasks to the Care Management Assistant
