Overview

• Travel/contract role in Home Health
• Location: Gloucester, Virginia
• Contract length: 13 weeks
• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Territory: Northern Neck
• Report location: Gloucester, VA
• Home health setting with home visits and periodic office meetings

Required Qualifications

• Valid Physical Therapist license in Virginia
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS
• Experience working with Adult patients
• Experience in a Home Health setting
• Driver's License

Preferred / Additional Details

• Home Health experience is the stated focus for this assignment
• First-timers not accepted
• Local candidates accepted at the same rate
• Weekend and on-call expectations vary by location and profession
• Some western territory positions may use a Baylor shift schedule of 4x10s, Friday–Monday
• Standard schedule for most positions is 5x8s, Monday–Friday

Assignment Details

• Average patient load: 5–6 patients per day
• Productivity expectation: 30 points per week
• Travel expectation: about 200 miles per week
• EMR: HCHB
• Types of cases: Start of Care and Follow Ups
