• 40 hours per week
• M-F 9:00 to 5:30, variable hours

Contract Length

• 13 weeks

Required Qualifications

• Valid Physical Therapist license in Illinois
• Physical Therapist experience within the last year
• At least 1 year of experience as a Physical Therapist
• Bachelor's degree in physical therapy
• BLS issued by American Heart Association

Job Summary

• Evaluates and treats patients of all ages, modifying the plan of care as appropriate
• Serves as a role model in delivery of professional services and as a clinical resource for staff and students

Key Responsibilities

• Performs patient transfers with assistance as needed from bed, chair, and floor, if applicable
• Uses safe body mechanics to reduce risk of injury
• Designs and adjusts the plan of care based on patient goals, response, and available evidence
• Educates patients, families, caregivers, students, and other health care providers
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care
• Performs patient examinations and screening
• Produces documentation in accordance with the Clinical Process Policy
• Uses and monitors support personnel according to legal and ethical guidelines
• Completes annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements, if applicable
• Evaluates impairments related to orthopedic and neurologic conditions
• Completes objective testing for range of motion, strength, and daily functional tasks
• Completes daily documentation and charging in therapy EMR
• Applies tape for stabilization, facilitation, or inhibition of movement
• Performs therapeutic modalities as appropriate, including ultrasound, interferential, and iontophoresis
• Communicates with the referring provider on patient progress and treatment recommendations
