Job Description

Role: Home Health Physical Therapist (PT)
Start Date: ASAP
Shift: 8-hour days (Note: 4x10s or 3x12s are not accommodated)
Patient Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
Experience Required: 1 year of skilled PT experience (willing to accept clinicians without prior home health experience)
Weekend Requirement: 1 weekend shift per contract
Certifications Required: BCLS
On-Call Requirement: No
Floating Requirement: No
Local Candidates Accepted: Yes (within a 50-mile radius)
Pending License Accepted: Yes
Guaranteed Hours: Facility may call off traveler up to 1 shift per pay period

Special Requirements

Transportation: Traveler is expected to have their own transportation.
Client Grouping: The system minimizes drive and windshield time, with occasional maximum radius of 50+ miles.
Oasis Experience: Preferred but not required.
