Job Description
- Role: Home Health Physical Therapist (PT)
- Shift: 8-hour days (Note: 4x10s or 3x12s are not accommodated)
- Patient Ratios: 1:1 (may visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: 1 year of skilled home health experience
- First-Timers Accepted: No
- Weekend Requirement: 1 weekend shift per contract
- Certifications Required: BCLS
- On-Call Requirement: No
- Floating Requirement: No
- Local Candidates Accepted: Yes (within a 50-mile radius)
- Pending License Accepted: Yes
- Guaranteed Hours: Facility may call off traveler up to 1 shift per pay period
Special Requirements
- Transportation: Traveler is expected to have their own transportation.
- COVID Vaccination: Primary series COVID vaccination plus one booster required.
- Oasis Experience: Preferred but not required.
