Job Description

This position is a replacement role due to the immediate end of a previous contract. The productivity metrics are as follows:

• Productivity: 6 weeks; 121% YTD: 99%
• Agency FTE: 5.9
• RN Vacancy Rate: 35.7%
• ESS FTE: 1.8

The facility also supports virtual nursing, which will reduce staffing needs by 2.7 FTEs.

Required Credentials

• BLS Certification: Required for clinical positions (American Heart Association only).
• ACLS Certification: Required for clinical positions (American Heart Association only).
• License/Certificate Verification: Primary source verification of professional license and/or certification, updated every 2 years.
• MMR: Positive titers only from within the last 15 years.
• Fit Test: Completed within 1 year of start.
• Hepatitis B: Proof of vaccination series or signed declination.
• Varicella: Positive titers only from within the last 15 years.
• Tdap: Proof of vaccination within the past 10 years.
• NIHSS Certification: Required.
• UHS I-9 Attestation: Completed I-9 or attestation required.
• UHS Ohio Fingerprints: Required for background check.
• UHS Education Verification: Highest level of education verified.
• UHS Employment Verification: Verification of employment within the last 5 years.
• UHS Skills Checklist: Skill-specific checklist required.
• UHS Competency Exam: Must score at least 80%.
• UHS Core Competencies: Must complete within 1 year prior to start.
• Physical/Health Statement: Pre-employment exam statement required.
• Influenza Vaccine: Proof required during flu season.
• 2-Step PPD or IGRA: Required annually.
• UHS Submission Packet: Resume, education, work history, and references required.
• UHS Badge Photo: Headshot required.
• UHS Criminal Background Check Results: Must be completed within 6 months of start.
• UHS OIG Search: Must be completed within 6 months prior to start.
• UHS SAM/EPLS/GSA/OFAC: Supporting documentation required.
• UHS Drug Screen: Must be completed within 60 days of start.
• UHS Module Completion Certificate: Safety and compliance training required.
