Job Description

• Experience Required: Minimum of 2 years; first-time travelers are not accepted.
• Skills Required:

  • Outpatient Clinic
  • Assessment, Discharge, Evaluation, Plan of Care, Reassessments
  • Medicare Documentation
  • Family Education
  • Communication Board
  • Experience with CVA, TBI, Fluency, Voice, Screenings, Hearing Impairments, Bedside Swallow Evaluation, and various neurological conditions.
• Certifications Required:

  • BLS (Basic Life Support)

Preferred Skills and Certifications

• Transfer Training
• Experience with Multiple Sclerosis, Augmentative Communication, and various communication devices
• Familiarity with EPIC EMR system

Work Schedule

• Shift: 40 hours per week
• Days:

  • Monday & Wednesday: 9 AM - 7 PM
  • Tuesday, Thursday & Friday: 8 AM - 3:30 PM
• Call Requirement: None
• Weekend Requirement: None

Department Details

• The Wellness Center is a multi-discipline clinic that includes PT, OT, and SLP services.
• Patient demographics range from infants to geriatrics, with a focus on neuro, geriatrics, orthopedics, and aquatics.

Additional Information

• Background Checks: Required within 30 days of start, including Social Security Verification and Criminal Background Checks.
• Health Documents:

  • Physical Exam within 1 year prior to start
  • Fit Mask Test
  • Vaccination records for Tdap, Measles, Mumps, Rubella, Hepatitis B, and Varicella
  • Negative TB test or Chest X-Ray
  • Drug Testing (10 panel) within 14 days prior to start
• References: Two current professional references required.

Compliance and Documentation

• Various authorizations and certifications are required, including but not limited to:

  • ACLS, PALS, TNCC, and others as specified.
• Compliance with facility-specific documents and health requirements is mandatory.
