Physical Therapist - Contract Position

Location: San Ramon, California

Job Description

• Provide physical therapy services within assigned county territories including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville.
• Additional travel outside the assigned area may be required.
• Weekend coverage may be required.

Work Expectations

• Guarantee Option:

  • Minimum productivity of 27 points per week (points correspond to visit types).
  • 36 hours guaranteed per week.
  • Must complete at least 30 points weekly.
  • Willingness to travel outside the area or work weekends to meet visit expectations.
  • Refusing patients voids the guarantee.
  • Guarantee does not apply during the first and last week of assignment, holidays, or approved time off weeks.

• Non-Guarantee Option:

  • No guaranteed hours.
  • May be required to travel outside the service area occasionally to secure a full caseload.
  • Can refuse patients.
  • Must be willing to see COVID-positive and COVID PUI patients.

Visit Point System & Hours Approval

• Visits are approved on a per-visit basis, with points assigned as follows:

  • OASIS ReCertification, Routine, Evaluation, Discharge, Follow-up, Re-Evaluation, etc. = 1 point = 1.33 hours
  • OASIS Start of Care (SoC) = 1.5 points = 2 hours
• Hours approved cover drive/travel time, visit time, documentation, and communication with patients and doctors.

Required Credentials

• Current California Physical Therapist License
• BLS Certification
• Valid Driver’s License, Auto Insurance, and Auto Registration
• Immunizations and screenings: TB (annual), MMR, Varicella, Hepatitis B, TDap (within 10 years), Flu (annual), Drug Screen, Physical Exam

Important Notes

• Agency contracts must exactly match the confirmed start and end dates.
• All requested time off (RTO) must be disclosed at submission; post-confirmation time off requests will not be approved.
• Non-compliance with contract or time off policies may result in removal from the client’s vendor list.
