Home Health Physical Therapist - Sacramento, CA

Facility: Mercy Home Health
Location: Sacramento, California
Contract Length: 13 weeks
Start Date: 6/24 (Leave of Absence coverage)

Qualifications & Requirements

• Minimum 2 years of healthcare experience; community/home health experience preferred
• Active California Physical Therapy License required
• Current BLS certification required
• Active NPI number required
• Valid driver's license and proof of automobile insurance required
• EMR experience with HomeCare Home Base (HCHB) preferred
• Experience with OASIS documentation required
• Candidates must not have been employed (full-time, part-time, or PRN) by any CommonSpirit-associated facility (including CHI or Dignity) within the past year; PRN employment through outside firms is acceptable
• Two supervisory references required at submission (one within 12 months, one within 3 years)

Job Description

• Provide physical therapy services according to a physician’s plan of care in a home health setting
• Assess and evaluate patients’ therapeutic, rehabilitative, and functional status
• Evaluate home environment and assess muscle strength, mobility, gait, and range of motion
• Direct physical therapy treatment and instruct patients, families, and caregivers on therapeutic appliance use and care
• Report patient reactions and changes in condition to the physician
• Initiate physical therapy programs and instruct other personnel or family members involved in patient care
• Train patients in the use of prosthetic devices as needed
• Supervise homemakers and Home Health Aides participating in rehabilitation
• Identify and refer patients/families for additional home health services as necessary
• Prepare and submit clinical and progress summaries based on patient goals and agency policy
• Provide physical therapy consultation and inservice education to nursing agency personnel
• Participate in peer consultation and ongoing staff competency assessments
• Comply with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other relevant regulations and licensure requirements
• Perform other duties as assigned

Schedule

• Monday to Friday, 8:30 AM to 5:00 PM
• Rotating weekends, typically 1 weekend every 5 weeks

• Mileage reimbursement available per GSA guidelines; mileage must be documented and approved by home health supervisors
• Timesheets require supervisor signatures and must be submitted according to agency procedures
