Required Certifications

• ACLS (Advanced Cardiac Life Support): AHA or ARC only; required for Cath Lab and other critical care areas
• BLS (Basic Life Support): AHA or ARC only; required for all candidates except some allied health roles
• NIH Stroke Certification: Required for all RNs except those at Le Bonheur Children's Hospital; expires every 2 years
• State License: Tennessee license or multistate license required; must be current and verifiable

Required Competencies & Exams

• Medication Test/Pharmacology Competency Exam: Must be taken within 1 year of start and updated annually
• Core Competency Exams (Parts I, II, III): Required annually for Case Managers (may be relevant depending on role)

Health & Immunization Requirements

• Physical Exam: Within 1 year of start, attesting ability to meet job demands
• Fit Mask Test: 3M N-95 mask fit test within 1 year, must pass without limitations
• Negative TB Test or Chest X-Ray: Within 1 year, updated annually
• Immunizations:

  • Measles (Rubeola), Mumps, Rubella: Proof of 2 vaccines or positive antibody titer; boosters required if titers low
  • Hepatitis B: Proof of 3-shot series, positive titer, or signed declination
  • Varicella: Proof of 2 vaccines or positive antibody titer; boosters required if titers low
  • Tdap: Required only for candidates working at Le Bonheur Children's Hospital
• Flu Vaccine (2024-2025 season): Required prior to start; no declinations accepted except approved religious or medical exemptions with documentation and possible mask requirement during flu season
• COVID-19 Vaccine: Preferred but not required

Background & Compliance Checks

• Background Check: Within 60 days of start, including statewide, county, federal, and education verification
• EPLS/GSA/SAM and HHS/OIG Checks: Required within 30 days of start and annually thereafter
• References: Two recent professional references required
• Education Verification: Required; diploma/transcript not accepted in lieu
• Sex Offender Search and Tennessee Adult Abuse Registry Search (for TN facilities): Required

• Photo for Badge: Must meet strict guidelines (passport-style, no hats, no glasses glare, recent within 6 months)
• Dress Code:

  • Navy blue scrubs for RNs
  • No prints or embroidered scrubs allowed
  • Business casual with white lab coat for RN Case Managers
• Confidentiality Acknowledgment: Must be signed prior to start
• Onsite Fit Test: Must be completed and brought on first day if applicable
• Job Description Form: Must be signed within one year of start
