Job Opportunity: RN CVOR (Long-Term Assignment)

Required Credentials and Certifications

• Driver's License/State ID/Passport (Required for onboarding)
• Prime OIG Clearance
• Prime Drug Screening: Minimum 10-panel test including THC (within 60 days of start)
• Prime Color-blind Test: Must be completed annually
• Prime Agency Attestation
• Prime SAM/EPLS Check
• Prime Resume (Required for submission)
• Prime References (2 required)
• BLS Certification: American Heart Association or Red Cross
• ACLS Certification: American Heart Association or Red Cross
• Nursys License Verification
• License/Certificate Verification: Primary source verification within 30 days of start
• Prime Education Verification
• COVID-19 Vaccine: 2 doses plus booster or completed declination
• MMR Vaccination: Proof of 2 vaccines at least 28 days apart or positive titer
• Hepatitis B Vaccination (Declinations not accepted)
• Influenza Vaccine: Required or surgical mask mandate during flu season
• TB Test: Quantiferon, T-Spot, or two negative PPD skin tests within past year
• Varicella Vaccination: Proof of 2 vaccines or positive titer
• Tdap Vaccination (Declinations not accepted)
• Physical/Health Statement: Must be completed within 60 days of start, stating clear to work
• Prime Core Competencies Exam: Must score 80% or higher
• Prime Skills Checklist
• Prime Criminal Background Check: National check within one year of start date
• Prime Form I-9: Employment eligibility verification
• Prime Specialty Exam: Must score 80% or higher
• Prime Orientation Packet: Includes "Essentials" form and post-test
• Prime Job Description: Signed by employee

Optional Credentials

• Name Verification Affidavit: For any name discrepancies between credentials
• Fit Test: Annual respirator fit test (optional)
• Prime Badge Photo: Clear headshot (JPEG) with plain background
