Travel Physical Therapist - Home Health

• EMR System: HCHB (Homecare Homebase)

Requirements

• Experience: Minimum 1 full year of recent adult Home Health experience
• First-Timers: Not accepted; must have recent Home Health experience
• Certifications: CPR and BLS required
• Licensure: Must have license in hand (pending licenses not accepted unless license is expected within a few days)
• Transportation: Reliable transportation and current driver's license required
• Insurance: Agency-required liability insurance required
• Local Candidates: Accepted at the same rate
• Points System: Contractor assigned 5-10 patients per day equating to roughly 6-8 points daily
• Guaranteed Hours: No, but options to float to nearby clinics if patient census is low

• Must complete various compliance, privacy, and policy trainings prior to start
• Required to review and sign multiple facility-specific documents annually if working consecutively for more than one year
• Background checks, drug screening, immunizations, and health documents required per agency standards
• Mileage reimbursed at IRS standard rate (patient-to-patient travel is billable; commute to/from first and last patient is not)
