Job Opportunity: Physical Therapist (PT) - Travel/Contract
Location: Terre Haute, Indiana
Facility: Amedisys - Terre Haute
Specialty: Therapy/Rehabilitation (Home Health)
Contract Length: 13 weeks (typical)
Job Description
- Provide Physical Therapy services in a Home Health setting with patient home visits.
- Average caseload: 5-6 patients per day.
- Types of visits: Start of Care, Follow Ups.
- Documentation, travel, and other activities are assigned point values; expected to achieve about 30 points per week.
- Schedule: Monday to Friday, 8 AM to 5 PM.
- Weekend rotation: None.
- On-call requirements: Varies by location; discuss during interview.
- Must be willing to travel up to 75 miles from the home clinic; weekly travel estimated between 100-300 miles.
- Local candidates accepted at the same rate.
Requirements
- Minimum 1 full year of recent Adult Home Health experience (first-timers not accepted).
- Current Physical Therapy license in hand (pending licenses not accepted unless only a few days to obtain).
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Reliable transportation and current driver's license required.
- Agency-required liability insurance required.
- Experience with Home Health EMR systems: HCHB and PointCare preferred.
- Must be able to attend periodic office meetings.
Productivity Expectations
- 30 points weekly (points assigned based on patient visits and related activities).
Coverage Area
- Must cover any territory within a 7-county radius around Terre Haute, Indiana.
Compliance and Documentation Requirements
Candidates must complete and maintain the following prior to and during assignment:
Certifications & Training
- CPR/BLS certification (current).
- OASIS E Course (if no recent experience with HCHB EMR).
- Compliance training and quizzes (including Code of Conduct, HIPAA, Privacy Policy, COVID-19 Policy, and others).
- Home Health Core Policy Acknowledgement and related quizzes.
Licenses & Background Checks
- Valid PT license for Indiana.
- Standard background checks covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start.
- Fingerprinting as required by state and facility policies.
- Work history verification for the last 2 years.
- Education verification for applicable degree.
Health & Immunization
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel) per Aya Healthcare standards.
- Seasonal flu vaccination or declination documentation.
- COVID-19 vaccination series completion or facility declination.
Insurance & Other Documents
- Personal or auto insurance policy (must be current and under candidate’s name or list candidate as a driver).
- Agency-required liability insurance.
- Various state-specific and facility-specific attestations, releases, and acknowledgements as applicable.
Additional Notes
- No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.
- Must be able to complete all required documentation and compliance packets prior to start and annually if assignment extends beyond one year or after any break in service.
- Some positions may have alternative shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
This position is ideal for experienced Physical Therapists with recent home health experience who are comfortable with travel and working independently in patient homes within the Terre Haute, Indiana area.
