Job Description
Seeking a full-time Physical Therapist in a hospital setting with a focus on Outpatient Rehab and Inpatient coverage.
Required Certifications
- California Physical Therapist License
- BLS (Basic Life Support) Certification
Job Duties
- Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions from disease or injury.
- Build confidence and cooperation from patients, families, and healthcare providers through competent assessment, monitoring, care, and communication.
- Adhere to all local, state, and federal regulations to ensure privacy and safety while delivering optimal patient care.
- Provide clinical supervision of assistants and aides; serve as clinical instructor for assigned students.
Rehabilitation Services Responsibilities
- Design and implement individualized physical treatment programs to maintain, improve, or restore physical function and alleviate pain.
- Complete and document patient evaluations and tests according to standards and regulations.
- Develop and update patient Plans of Care based on assessments and diagnoses.
- Provide and progress treatment per Plan of Care.
- Complete all documentation related to charge capture.
- Coordinate patient care with other providers and support team members.
- Work flexible shifts (days of week, hours).
- Provide clinical supervision and serve as clinical instructor.
Clerical Duties
- Prepare and complete accurate documentation of patient therapy in medical and departmental records.
- May perform general clerical tasks such as answering phones, updating appointments, and managing electronic health records (EHR).
Safety and Compliance
- Participate in quality improvement and interdisciplinary projects.
- Maintain a clean, safe working environment; clean and disinfect equipment after use.
- Inspect and use equipment per manufacturer guidelines; report malfunctions.
- Maintain strict confidentiality of all patient Protected Health Information (PHI).
- Prepare, administer, and document services in compliance with state and federal regulations.
- Retain certifications and complete mandated continuing education to meet accreditation and licensure requirements.
Education
- Graduate of an accredited physical therapy graduate program

- ACLS (Advanced Cardiac Life Support)
- PALS (Pediatric Advanced Life Support)
- NRP (Neonatal Resuscitation Procedure)
- CCRN (Critical Care RN)
- TNCC (Trauma Nursing Core Course)
- EFM (Electronic Fetal Monitoring)
- Management of Assaultive Behavior (MAB)
- CPI (Crisis Prevention Institute)
- NIH Stroke Certification
- Medication/Pharmacology Competency Exams (as applicable)
Facility Specific Requirements
- Review and sign facility policies including Standards of Business Conduct, Code of Conduct, Vendor Training, and others prior to start.
- TB Screening Questionnaire and annual renewal required.
- Physical exam within 1 year of start.
- Fit mask test within 1 year, renewed annually.
- Immunizations and titers required: Tetanus/Diphtheria/Tdap, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, Flu seasonal vaccination during flu season.
- Negative TB test or chest X-ray within 90 days of start, renewed annually.
- Drug screening (9 panel) within 6 months of start.
- Color Blind Exam (in-person) if applicable.
- Respiratory Safety clearance prior to start, renewed annually.
Background and History Checks
- 7-year background check within 6 months of start.
- EPLS/GSA/SAM, HHS/OIG checks updated annually.
- Sex Offender Search within 90 days of start.
- FACIS Level 3 screening within 2 weeks of start and monthly thereafter.
- Employment application with full 7 years of work history, no gaps.
- Education verification of highest level achieved.
- Work history verification for up to 7 years.
- References required by Aya Healthcare (not by Sutter Health).
