Job Description

Seeking a full-time Physical Therapist in a hospital setting with a focus on Outpatient Rehab and Inpatient coverage.

Required Certifications

• California Physical Therapist License
• BLS (Basic Life Support) Certification

Job Duties

• Assess, plan, organize, and participate in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and correct disabling conditions from disease or injury.
• Build confidence and cooperation from patients, families, and healthcare providers through competent assessment, monitoring, care, and communication.
• Adhere to all local, state, and federal regulations to ensure privacy and safety while delivering optimal patient care.
• Provide clinical supervision of assistants and aides; serve as clinical instructor for assigned students.

Rehabilitation Services Responsibilities

• Design and implement individualized physical treatment programs to maintain, improve, or restore physical function and alleviate pain.
• Complete and document patient evaluations and tests according to standards and regulations.
• Develop and update patient Plans of Care based on assessments and diagnoses.
• Provide and progress treatment per Plan of Care.
• Complete all documentation related to charge capture.
• Coordinate patient care with other providers and support team members.
• Work flexible shifts (days of week, hours).
• Provide clinical supervision and serve as clinical instructor.

Clerical Duties

• Prepare and complete accurate documentation of patient therapy in medical and departmental records.
• May perform general clerical tasks such as answering phones, updating appointments, and managing electronic health records (EHR).

Safety and Compliance

• Participate in quality improvement and interdisciplinary projects.
• Maintain a clean, safe working environment; clean and disinfect equipment after use.
• Inspect and use equipment per manufacturer guidelines; report malfunctions.
• Maintain strict confidentiality of all patient Protected Health Information (PHI).
• Prepare, administer, and document services in compliance with state and federal regulations.
• Retain certifications and complete mandated continuing education to meet accreditation and licensure requirements.

Education

• Graduate of an accredited physical therapy graduate program

• NRP (Neonatal Resuscitation Procedure)
• CCRN (Critical Care RN)
• TNCC (Trauma Nursing Core Course)

• Management of Assaultive Behavior (MAB)
• CPI (Crisis Prevention Institute)
• NIH Stroke Certification
• Medication/Pharmacology Competency Exams (as applicable)

Facility Specific Requirements

• Review and sign facility policies including Standards of Business Conduct, Code of Conduct, Vendor Training, and others prior to start.
• TB Screening Questionnaire and annual renewal required.
• Physical exam within 1 year of start.
• Fit mask test within 1 year, renewed annually.
• Immunizations and titers required: Tetanus/Diphtheria/Tdap, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, Flu seasonal vaccination during flu season.
• Negative TB test or chest X-ray within 90 days of start, renewed annually.
• Drug screening (9 panel) within 6 months of start.
• Color Blind Exam (in-person) if applicable.
• Respiratory Safety clearance prior to start, renewed annually.

Background and History Checks

• 7-year background check within 6 months of start.
• EPLS/GSA/SAM, HHS/OIG checks updated annually.
• Sex Offender Search within 90 days of start.
• FACIS Level 3 screening within 2 weeks of start and monthly thereafter.
• Employment application with full 7 years of work history, no gaps.
• Education verification of highest level achieved.
• Work history verification for up to 7 years.
• References required by Aya Healthcare (not by Sutter Health).
