Description
Job Opportunity: Therapy/Rehabilitation - Travel/Contract
Location: Live Oak, California
Facility: River Valley Care Center
Hospital System: Interface Rehab
Employment Type: Travel/Contract
Assignment Length: 13-26 weeks preferred
Start Date: ASAP (flexible for later starts)
Work Week Guarantee: 36 hours
Cancellation Notice: 14 days
Schedule
- 5 x 8-hour day shifts
- May include occasional weekend coverage as needed
Requirements
- Minimum 1 year of experience in Long-Term Care (LTC) / Long-Term Acute Care (LTAC) specialty preferred
- New graduates will be considered
Certifications & Licenses Required
- ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
- BLS (Basic Life Support) from American Heart Association or American Red Cross (current, clear copy required)
- PALS (Pediatric Advanced Life Support) preferred
- State License verification (primary source online verification)
- National Tech Certification as applicable (note some states require state licenses for techs)
Competencies
- Medication Test/Pharmacology Competency Exam (as applicable by profession)
- Competency Exam (as applicable by profession)
Health & Immunization Documents
- Physical exam within 1 year of start
- Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year of start; if past positive, negative chest X-ray within 1 year required
- Proof of immunity or vaccination for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, 2 vaccines, or history of chickenpox)
- 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
- Seasonal Flu Vaccination or Declination (required during flu season: 10/1 - 3/31)
Background & Compliance
- Standard background check (7-year search including SS trace, county searches, nationwide sex offender) updated annually
- EPLS/GSA/SAM checks (annual)
- HHS/OIG checks (annual)
- 2 Professional References required for Travel, Allied, Per Diem Acute Care; 1 for Per Diem Non-Acute Care (within 3 years of start)
- Annual Skills Checklist
Additional Notes
- Emergency Contact Form required prior to start
- Government issued photo ID (Driver’s License/State ID) required
