Job Description

  • Position: Cath Lab Technician
  • First and local OK
  • 13-week assignment

Required Certifications

  • ACLS (Advanced Cardiac Life Support) – ARC, AHA, or DoD approved
  • BLS (Basic Life Support) – ARC, AHA, or DoD approved
  • PALS (Pediatric Advanced Life Support) – as applicable
  • NRP (Neonatal Resuscitation Procedure) – as required
  • TNCC (Trauma Nursing Core Course) – as required
  • State License – Primary Source Verification required
  • NURSYS Verification (for RNs)
  • HIPAA Safety Manual Acknowledgement
  • Tech Certification Verification (national) – refer to Allied Licensure Report; not required for CVOR Techs

Required Competencies

  • Medication Test/Pharmacology Competency Exam – as applicable
  • Unit Competency Exam – must be updated annually
  • Core Parts I, II, III (for subcontractors) – HIPAA form or core testing acceptable

Facility-Specific Documents (Blanchard Valley Hospital)

  • Contract Employee Checklist (must sign and date)
  • Orientation to Our Work Environment (must review and sign)
  • Badge Authorization Form (must complete and sign)
  • Confidentiality Agreement (must review and sign)
  • System Access Form (required prior to start)
  • COVID-19 Exemption Attestation (for approved medical/religious exemptions)
  • Flu Declination Form (if declining flu vaccine; deadline 12/1/2024)
  • COVID-19 Unvaccinated Education Acknowledgement (if declining COVID vaccine)
  • Hepatitis B Declination Form (if declining Hep B vaccine or titer)

Health Requirements

  • Physical Exam – initial and annual update attesting ability to meet physical demands
  • Negative TB Test or Chest X-Ray – within 1 year of start; annual TB questionnaire required
  • Immunizations or Titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (vaccine, titer, or declination)
    • Varicella (vaccine, titer, or proof of disease)
  • Drug Testing – 9 or 10 panel drug screen within 30 days of start; not required annually during assignment
  • Seasonal Flu Vaccination or Declination (must comply by 12/1/2024)
  • COVID-19 Vaccine – fully vaccinated or approved exemption required; accommodations apply for exemptions

Background and History Checks

  • 7-year criminal background check within 30 days of start
  • EPLS/GSA/SAM checks within 30 days of start and updated with contract renewals
  • HHS/OIG checks within 30 days of start and updated with contract renewals
  • Proof of 2 clinical or professional references
  • Hospital evaluation if worked over 1 year with the system
  • Internal evaluation if worked over 2 years with the system
  • Education verification required
  • Skills checklist updated annually

Identification and Legal Documents

  • Proof of I9 or attestation
  • Government-issued photo ID (Driver’s License or State ID) required and must be carried during work hours
  • Proof of E-Verify or attestation
  • Residency Declaration Form (if working on a compact license)

This position requires compliance with all listed certifications, health requirements, background checks, and facility-specific documentation prior to and during the assignment.
