Registered Nurse (RN) - Home Health Care

Location: Cheboygan, Michigan

Position Summary

Provide full-time professional nursing care to patients in their homes under supervision, following agency standards and policies.

Key Responsibilities

• Initiate and provide medically prescribed skilled nursing care and health counseling to support treatment, rehabilitation, and symptom management.
• Conduct initial patient visits to assess physical and emotional status, needs, and resources through observation, interviews, and record analysis.
• Develop nursing care plans aligned with physician treatment plans, including patient teaching based on Care Plan/Standard of Care.

Requirements

Experience & Skills

• Minimum 2+ years of nursing experience (required)
• Infusion and wound care experience (required)
• Oasis experience (preferred)
• Travel preferred
• Home Care Home Base experience (required)

Certifications & Licenses

• BLS (AHA) certification (required)
• Active Michigan nursing license (required at time of submission)

Coverage Area & Work Environment

• Coverage area includes Northern Michigan, based out of Petoskey branch, covering the "tip of the Mitten" region.
• Position involves significant driving.
• EMR system: Home Care Home Base
• Practice management: Point of Care
• Dress code: Matching scrubs
