Location: Anchorage, Alaska

Job Details

Required Certifications and Licenses
- Basic Life Support (BLS)
- Alaska State License (or pending accepted)
- Adult outpatient experience as per user data (UD)

- Physical Exam (within 1 year, signed by MD, NP, PA, or DO)
- Fit Mask Test (annual, onsite if no recent record)
- Tetanus/Diphtheria/Tdap (within 10 years or declination)
- Negative TB Test or Chest X-Ray (annual update required)
- MMR Vaccines or Positive Titers for Rubeola, Mumps, Rubella
- Hepatitis B vaccination or signed declination (Alaska placements require positive titer and vaccine series)
- Varicella vaccination or positive titer
- 10 Panel Drug Screen (within 90 days)
- Seasonal Flu Vaccine or Declination (required during flu season)
- COVID-19 Vaccine or Declination (required if onsite)
- Respirator Safety Clearance (annual for travel candidates)
Background and History
- Background Check (within 45 days, covering last 7 years)
- EPLS/GSA/SAM checks (within 30 days, monthly updates)
- References: Minimum 2 supervisor references covering 1 year in relevant specialty
- Employment Application: Last 5 years of employment history required
- Skills Checklist (annual update if applicable)
