Position Details

- Competency exams and medication/pharmacology tests as applicable by profession
Compliance & Onboarding Requirements
- Background Check: Within 45 days of start, covering all locations lived/worked in past 7 years; updated annually
- Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)
- Health Documents:
- Physical exam within 1 year of start
- Immunizations: TDAP, MMR (Measles, Mumps, Rubella), Hepatitis B (or declination), Varicella
- Negative TB test or chest X-ray within 12 months, updated annually
- Flu vaccine during flu season (declinations accepted)
- COVID-19 vaccine or declination if onsite
- Fit mask test (N95 or approved equivalent), updated annually for travel staff
- Respiratory safety clearance for respirator use (travel only)
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within last 3 years
- Employment Application: Must include at least last 5 years of employment with explanations for gaps/overlaps
- Education Verification: Required for all candidates; international education requires additional documentation
- Skills Checklists: Required annually for applicable units
