Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- BLS (Basic Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- PALS (Pediatric Advanced Life Support) – AHA or Canadian Heart & Stroke Foundation certification
- NRP (Neonatal Resuscitation Program) – Current certification from AAP, AHA, or Canadian Paediatric Society
- EFM (Electronic Fetal Monitoring) – AWHONN or NCC-EFM certification (or equivalent as approved)
- Chemotherapy certification (if applicable)
- CPI (Crisis Prevention Institute) – if required by unit
- IV Certification (if applicable)
- Management of Assaultive Behavior (MAB) – if required
- NIH Stroke Certification – if required
- State Nursing License verification (NURSYS verification required for RNs)
Required Competencies
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exams relevant to specialty (including Fetal Heart Rate Monitoring Assessment or EFM certification for L&D RNs)
- Color Blind Testing (if required by unit)
Health and Safety Requirements
- Physical Exam within 1 year of start (signed by MD, NP, PA, or DO)
- Fit Mask Test (N95 or equivalent) – must be current within 1 year; annual updates required
- Tetanus/Diphtheria/Tdap vaccine within 10 years (declination accepted)
- Negative TB test or Chest X-Ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- 10 Panel Drug Screen within 90 days of start (no instant tests; THC/marijuana excluded)
- Seasonal Flu Vaccine (declination accepted with mask requirement)
- COVID-19 Vaccine or Declination (required if onsite)
- Respiratory Safety Clearance (RespSafety questionnaire and clearance annually for travel)
Background and Compliance
- Background Check within 45 days of start covering all locations lived/worked in past 7 years; updated annually
- EPLS/GSA/SAM checks within 30 days of start; updated monthly
- References: Minimum 2 supervisor references covering 1 year of employment in comparable specialty within last 3 years
- Employment Application covering at least last 5 years of employment
- Education Verification (nursing degree verification required)
- Skills Checklist updated annually (applicable units)
- State-specific searches and attestations as required for Washington and Oregon placements

- Floating Policy Acknowledgement required (may float to other Providence facilities nearby)
- Badge photo required (passport style with neutral background)
- Completion of Providence WorkRamp training modules prior to start (Core, Specialty, Regional, and Annual Regulatory modules as applicable)

Additional Information
- 2-year and 1-year renewals required for certain background and health documents
- Some certifications and competencies may be deferred or adjusted based on clinical track or unit requirements
- Non-clinical and remote workers may have reduced requirements
