Description
Job Opportunity: Therapy/Rehabilitation - Travel/Contract
Location: South Bend, Indiana
Facility: Memorial Hospital of South Bend
Hospital System: Beacon Health System
Contract Length: 13 weeks
Required Certifications
- ACLS (Advanced Cardiac Life Support) – AHA only
- BLS (Basic Life Support) – AHA only
- PALS (Pediatric Advanced Life Support) – AHA only
- NRP (Neonatal Resuscitation Procedure)
- CHEMO (Chemotherapy)
- TNCC (Trauma Nursing Core Course)
- EFM (Electronic Fetal Monitoring)
- Tech Cert Verification - National (as applicable per profession)
- State License Verification (as applicable per profession)
- NIH Stroke Certification
- Pass/Fail Testing (EKG/Medication) – Required for Monitor Techs

Health and Compliance Requirements
- Physical Exam – Required prior to start
- Negative TB or Chest X-Ray – Negative 2-step PPD or Qgold within 1 year; T-Spots not accepted
- TB Questionnaire – Required prior to start and annually
- Immunizations:
- Rubeola (Measles) – Titer or 2 vaccines
- Mumps – Titer or 2 vaccines
- Rubella – Titer or 2 vaccines
- Hepatitis B – Titer, series, or declination
- Varicella – Titer or 2 vaccines
- Drug Testing – Negative 12-panel drug screen within 30 days of start (instant accepted if needed for quick start)
- Flu Seasonal Vaccination/Documentation (2024-25) – Vaccine or approved medical/religious declination required prior to start; documentation must be on file by 12/15 for working candidates
- COVID-19 Medical Exemption – Medical exemption requests must be submitted with healthcare provider documentation if applicable
Background and Education
- Background Check – Within 90 days of start, covering last 7 years and all states lived/worked; includes SSN trace, criminal, sex offender, and motor vehicle report (for phlebotomy)
- Education Verification – Copy of diploma or transcripts required
Facility Specific Requirements
- Photo for Badge – Clear, passport-style photo with solid background (.jpeg format)
- Confidentiality and Non-Disclosure Agreement – Required prior to start
- Agency Resource Guide – Must be reviewed and receipt signed
- Flu Medical and Religious Declinations – Must be submitted and approved if applicable
- Modules – Completion of facility-specific training modules required prior to start (8-10 hours)
- Acknowledgement of Pass/Fail Testing – On-site EKG exam required for Monitor Techs
- Beacon Health - Cardiac Rhythms Review – Required for Monitor Techs
