Job Description

• Provide Speech-Language Pathology services in a home health setting.
• Conduct home visits with an average of 5-6 patients per day.
• Participate in periodic office meetings.
• Manage patient caseloads of 5-10 patients per day.
• Types of cases include Start of Care and Follow-Ups.
• Use the HCHB electronic medical record system.
• Coverage area includes San Francisco to San Mateo (Peninsula), with possible travel to San Jose, Campbell, and Marin County.
• Expected to travel approximately 150 miles per week, up to 75 miles from the home clinic.
• Local candidates are accepted.

Requirements

• Experience: Minimum 1 full year of recent Adult Home Health experience required. First-timers in home health are not accepted.
• Certifications: Current CPR and BLS certification required.
• Licensure: Must have a valid license in hand (pending licenses accepted only if they can be obtained within a few days).
• Transportation: Reliable transportation and a current driver’s license required.
• Insurance: Must have agency-required liability insurance.
• Schedule: Typically Monday-Friday, 8 AM to 5 PM. Weekend and on-call requirements vary by location and should be discussed during the interview.
• Workload: Increase in speech therapy referrals due to no available ST staff.
• Guaranteed Hours: No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.

Additional Information

• Must be willing to comply with all required background checks, drug screening, and health documentation.
• Required to review and sign various compliance, confidentiality, and policy documents prior to start.
• Must complete health requirements including physical exam, TB testing, immunizations (MMR, Hepatitis B, Varicella), and COVID-19 vaccination or declination.
• Fingerprinting and state-specific background checks are required as applicable.
• Must complete OASIS E course if no recent experience with HCHB within the last 6 months.
• Mileage reimbursement provided at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).

This is a travel/contract position for an experienced Speech-Language Pathologist specializing in adult home health therapy.
