Job Description

• Provide physical therapy services primarily through home visits, with some visits to care facilities.
• Average patient load: 5-6 patients per day.
• Types of cases: Start of Care and Follow-Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (some locations may vary).
• Travel: Must be willing to travel up to 75 miles from the home clinic; approximately 30-40 miles per day, 100 miles per week.
• Documentation and travel time between patients are part of the workload.
• EMR systems used: HomeCare HomeBase, Oasis, Outlook.
• On-call requirements vary by location; discuss during interview.
• Weekend requirements vary; discuss during interview.
• Potential for hire at the end of the contract.

Requirements

• Minimum 1 full year of recent adult home health physical therapy experience.
• First-time home health therapists not accepted.
• Current, valid physical therapy license required (pending licenses accepted only if license issuance is imminent).
• CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and current driver's license required.
• Agency-required liability insurance required.
• Must be able to meet productivity expectations (~25-30 weekly units).
• Must be able to complete all required documentation and compliance training prior to start.

Compliance and Documentation

Candidates must complete and maintain the following (as applicable):

• Valid Driver’s License/State ID.
• Background checks including 7-year history, sex offender registry, and state-specific searches.
• Drug screening (10-panel).
• Physical exam within 12 months confirming good health and no communicable diseases.
• Negative TB test or chest X-ray within 12 months.
• Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
• COVID-19 vaccination or documented declination.
• Flu vaccination or documented declination for the current season.
• Fingerprinting as required by state and facility.
• Completion of facility-specific compliance documents including HIPAA, confidentiality agreements, code of conduct, and home health/hospice policy quizzes.
• Completion of OASIS E course if no recent experience with HomeCare HomeBase EMR.
• Various state-specific forms and attestations depending on placement location.

Additional Notes

• Mileage reimbursement follows IRS standard rates; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
• Local candidates are accepted at the same rate.
• Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules.
• Candidates must be prepared to sign new supplier packets at the start of each assignment and annually if working consecutively for more than one year.
• Candidates should be prepared for potential floating to nearby clinics if patient census is low.
