Job Description
Provide oncology nursing care as a registered nurse in a travel assignment at Ascension Via Christi St. Francis in Wichita, KS.
Required Credentials & Compliance
Certifications & Licenses
- BLS (Basic Life Support): Must be American Heart Association (AHA) certified.
- Nursys Verification: Must be completed within 30 days prior to start. Disciplinary actions require FACIS Questionnaire.
- License/Certificate Verification: Current license and primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- MOAB Moderate Risk De-Escalation Training: Required.
- Driver's License/State ID: Must be valid, not expired, and match address requirements.
Testing & Competencies
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly. Minimum 80% score required.
- Ascension EKG Exam: Must pass with at least 80%, two attempts allowed.
- Ascension Cerner Test-Out: Required for electronic health record proficiency.
- Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.
- Ascension 12 Panel Drug Screen: Must be completed within 30 days prior to start and updated annually. Includes testing for 12 specified substances.
Onboarding & Professional Requirements
- Ascension Onboarding Requirements Education Attestation: Completion of pre-hire education modules and attestation required.
- Ascension Nursing Contract Worker Onboarding: Onsite completion required.
- Ascension Nova StatStrip: Pre-hire modules for non-allied positions.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension Profile: Resume or professional profile submission required.
- Ascension 2 Professional References: At least two professional references required prior to first assignment.
- Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check; updated annually or after breaks in service.
- Ascension EPLS (Excluded Parties List System): Must be run within 90 days of start and rerun if break in service > 90 days.
- Ascension Education Verification: Verification of relevant education within 90 days of start; official transcripts required if verification fails.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updated with extensions.
- Ascension Employment Verification: Third-party employment verification for past seven years; updated annually or after breaks in service.
Personal Health History & Immunizations
- Ascension Color Vision: Completed prior to start and updated every 3 years.
- MMR (Measles, Mumps, Rubella): Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite. Updated annually.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Optional but recommended; flu season is October 1 through March 31. Exemptions accepted with mask requirement.
- Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest x-ray within a year, and annual TB questionnaire.
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days prior to start; annual updates required.
- Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Ascension 12 Panel Drug Screen: See above under Testing & Competencies.
Optional / Additional Information
- Ascension Unit Specific Checklist: Optional for onboarding; must be uploaded by unit leader within a week.
- Hiring Manager Evaluation: Post-assignment evaluation (optional).
- Ascension Name Change Document: Legal documentation of name change (optional).
- Ascension Influenza Vaccine Exemption/Declination: Accepted with mask requirement during flu season.
