Job Opportunity: Interventional Radiology Technician

Location: Westerville, Ohio
Facility: Mount Carmel St. Ann's (Trinity Health)
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks


Work Schedule

  • Dayshift: 40 hours per week, 4 days of 10 hours each (4x10)
  • Typical hours: Monday to Friday, 6:30 AM to 5:00 PM (varied days)
  • Call: 14 days of call including 2 weekends
  • Call hours: Monday to Friday 4:00 PM to 7:00 AM, Saturday and Sunday 24/7

Required Credentials and Onboarding Requirements

Personal Health History

  • COVID-19 Vaccine: Proof of vaccine including September 2024 booster, or declination/exemption with agency letterhead (declination not accepted in CA or IL)
  • MMR: Proof of 2 vaccinations with dates or IgG titer lab report
  • Fit Test: Required (specific mask type for Iowa)
  • Hepatitis B: Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later). Titers not accepted.
  • Influenza Vaccine: Optional; proof or declination/exemption accepted (declination not accepted in CA or IL) for flu season 10/1/2025 - 4/30/2026
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within 30 days (PPD) or 90 days (IGRA) before start
  • Varicella: Proof of 1 vaccination or IgG titer lab report
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work
  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (includes methadone, phencyclidine, benzodiazepines, cocaine, propoxyphene, amphetamines, opiates, 6-acetyl morphine, methaqualone, barbiturates)

Professional Requirements

  • Confidentiality & Security Agreement: Signed Trinity Health confidentiality and system security agreement
  • Conflict of Interest Disclosure Form: Signed and on file
  • Criminal Background Check: Federal, nationwide, and county checks for last 7 years, including SSN verification, dated within 30 days of start
  • Education Verification: Highest academic degree or diploma/GED verified by third party screening
  • EPLS (Excluded Parties List System): Report dated within 30 days, all aliases searched
  • GSA/OIG Exclusion Check: Report dated within 30 days, all aliases searched
  • Integrity and Audit Services Form: Signed shortly before start
  • Joint Commission Testing: CORE exam within the last year
  • NSO (National Sex Offender) Check: Dated within 30 days, all aliases searched
  • System Access Form: Completed and uploaded one week before start
  • Skills Checklist: Provided by agency for clinical contractors
  • BLS Certification: American Heart Association BLS certification required, including front and back of card
  • License/Certificate Verification: Must be dated within 30 days of start, with online verification for required licenses/certifications
  • Code of Conduct Form: Signed and on file
  • I9 Form: Completed by agency
  • Specialty Specific Competency Testing: Specialty exam completed within the last year
  • Color Vision Test: Required

  • Resume, Skills Checklist (within 1 year), and 2 Professional References must be submitted
