Job Opportunity: Interventional Radiology Technician

Location: Columbus, Ohio

Work Schedule

• Dayshift: 06:30 AM – 04:00 PM
• On-call: 10-12 shifts over 6 weeks, including 2 weekends

Required Credentials and Onboarding Requirements

Personal Health History

• Fit Test: Required before onboarding
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Must be current within 30 days (PPD) or 90 days (IGRA) before onboarding
• Trinity Specific Covid Questionnaire: Must be completed and uploaded before onboarding
• COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption accepted (except CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report required
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Varicella: Proof of 1 vaccination or IgG titer lab report required
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Influenza Vaccine: Optional, but recommended (declination accepted except CA or IL)
• Trinity Color Vision: Required

Testing and Checklists

• Trinity Skills Checklist: Provided by agency for clinical contractors, required before onboarding
• Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), must include specific substances (NO THC)
• Trinity Specialty Specific Competency Testing: Specialty exam within the last year required

Professional Requirements

• Confidentiality & Security Agreement: Signed form required before onboarding
• Conflict of Interest Disclosure Form: Signed form required before onboarding
• Criminal Background Check: Federal, nationwide, county (last 7 years), SSN verification, dated within 30 days of start
• Education Verification: Highest academic degree or diploma/GED verified by third party screening
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start
• GSA/OIG Exclusion Check: Dated within 30 days of start
• Integrity and Audit Services Form: Signed shortly before start
• Joint Commission Testing: CORE exam within the last year
• NSO (National Sex Offender) Check: Dated within 30 days of start, including aliases and locations
• System Access Form: Must be completed and uploaded one week before start
• Code of Conduct Form: Signed form required before onboarding
• I9 Verification: Standard I9 completed by agency

Certifications and Licenses

• BLS (Basic Life Support): American Heart Association certification required, including front and back of card
• License/Certificate Verification: Must be dated within 30 days of start, with online verification for required licenses or certifications
