Job Opportunity: Home Health Speech-Language Pathologist (SLP)
Location: Novato, California (serving all Marin County within 50 miles)

Job Description
The Speech Therapist provides rehabilitation home therapeutic treatment and education to home health and hospice patients, coordinating care with other home health staff for integrated patient care.
Key Responsibilities
- Develop and implement treatment plans for speech and swallowing disorders (e.g., stuttering, delayed language).
- Conduct patient evaluations and tests according to Speech Pathology Standards of Practice.
- Develop and update patient Plans of Care (POC).
- Provide and progress treatment per established standards.
- Complete all required documentation accurately and timely.
- Coordinate patient care with other providers and support team members.
- Work flexible shifts (varied days and hours).
- Assist with clinical supervision of assistants and aides.
- Serve as a clinical instructor for assigned students.
Clerical Duties
- Prepare and complete accurate documentation of therapy performed in medical and departmental records.
- May perform general clerical tasks such as answering phones, updating appointments, and managing Electronic Health Records (EHR).
Safety and Compliance
- Participate in quality improvement and interdisciplinary projects.
- Maintain a clean, safe working environment; disinfect equipment after use.
- Inspect and use equipment per manufacturer guidelines; report malfunctions.
- Maintain strict confidentiality of all patient Protected Health Information (PHI).
- Administer and document services in compliance with state and federal regulations.
- Retain certifications and complete mandated continuing education to meet licensure and accreditation requirements.
Required Certifications and Licenses
- California Speech-Language Pathology (SLP) License
- Basic Life Support (BLS)
Additional certifications may be required or verified depending on position specifics, including but not limited to:
- Advanced Cardiac Life Support (ACLS)
- Pediatric Advanced Life Support (PALS)
- Neonatal Resuscitation Program (NRP)
- Other specialty certifications as applicable
Compliance and Documentation Requirements
Candidates must complete and maintain the following:
Health and Immunization
- Physical exam within 1 year of start.
- Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or facial hair).
- Immunizations or proof of immunity for:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola), Mumps, Rubella (MMR)
- Hepatitis B (series or declination)
- Varicella (chickenpox)
- Negative TB test or chest X-ray within 90 days, renewed annually.
- Seasonal flu vaccination or approved declination during flu season (Nov 1 - Apr 30).
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law).
Background and History Checks
- 7-year background check within 6 months of start, including SSN trace, county searches, sex offender search, education verification, and employment verification.
- FACIS Level 3 screening within 2 weeks of start and monthly thereafter.
- EPLS/GSA/SAM and HHS/OIG checks updated annually.
- References as applicable (Aya requirement, not client-mandated).
- Complete employment application with full 7 years of work history, no gaps.
Facility-Specific Documents
- Review and sign job description, standards of business conduct, code of conduct, vendor training, and other required facility policies prior to start.
- Complete conditional offer of employment attestation.
- Complete TB questionnaire within 90 days of start, renewed annually.
- Submit personal or auto insurance if position requires driving.
- Religious accommodation forms if applicable.
