Description

Occupational Therapist (OT) Position - Livonia, Michigan

Employment Type: Long-Term Assignment (LTA)
Location: Livingston and Washtenaw Counties, Michigan
Schedule: No weekends; may work some holidays (at least one)
System: EPIC electronic health record


Contract Length

  • 10 weeks

Required Credentials and Documentation for Onboarding

Certifications & Licenses

  • BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel.
  • License/Certificate Verification: Must be dated within 30 days of start; online verification required.
  • Driver's License/State ID: Valid photo ID with clear image of all corners and insurance card photo.

Health & Immunizations

  • Fit Test
  • Trinity 10 Panel Drug Screen: Completed within 30 days of start (preferred within 14 days), includes testing for multiple substances (NO THC).
  • COVID-19 Vaccine: Proof of September 2024 booster, declination or exemption accepted (except CA or IL).
  • MMR: Proof of 2 vaccinations or IgG titer lab report.
  • Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
  • Influenza Vaccine: Proof of vaccine, declination, or exemption for flu season 10/1/2024–4/30/2025 (declination not accepted for CA or IL).
  • 2 Step PPD OR IGRA (Quantiferon/Tspot): Positive TB requires chest x-ray within last year for Michigan.
  • Varicella: Proof of 1 vaccination or IgG titer lab report.
  • Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
  • Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction.

  • Integrity and Audit Services Form
  • Conflict of Interest Disclosure Form
  • Criminal Background Check: Federal, nationwide, county criminal check from last 7 years, including SSN verification, dated within 30 days of start.
  • Education Verification: Highest academic degree or diploma/GED verified by third party screening.
  • EPLS (Excluded Parties List System) Check: Dated within past 30 days.
  • Fingerprinting: State and department specific.
  • GSA/OIG Exclusion Check: Dated within past 30 days.
  • Joint Commission CORE Exam: Within the last year.
  • NSO (National Sex Offender) Check: Dated within 30 days; includes statewide check for Michigan.
  • System Access Form: Must be completed and uploaded one week before start.

Competency & Skills

  • Specialty Specific Competency Testing: Must be completed within the last year.
  • Skills Checklist: Provided by agency for clinical contractors.

This position requires thorough documentation and compliance with Trinity Health onboarding standards prior to start.
