Job Opportunity: RN Medical / Surgical / Telemetry (Nights)
Location: Port Huron, Michigan
Employment Type: Long-Term Assignment (LTA)
Shift: Nights, 7:00 PM – 7:30 AM
Specialty: Medical / Surgical / Telemetry Critical Care
**Contract Length: 12 weeks
Required Credentials and Certifications
Licenses & Certifications (Must be submitted prior to start)
- Registered Nurse License (Primary source verification within 30 days of start, updated upon expiration)
- BLS (American Heart Association or Red Cross)
- ACLS (American Heart Association or Red Cross)
- Nursys Verification (may be confirmed online)
- Driver’s License, State ID, or Passport
Health & Immunizations (Required for Onboarding)
- COVID-19 Vaccine: 2 doses plus booster or completed declination
- MMR: Proof of 2 vaccines or positive titer showing immunity
- Hepatitis B: Vaccination required; declinations not accepted
- Influenza Vaccine: Required during flu season (Nov 1 – Mar 31); declinations not accepted
- TB Test
- Varicella: Vaccination or titer showing immunity
- Tdap: Required; declinations not accepted in some states
- Physical/Health Statement: Must be current (within 60 days of start), stating clearance to work
- Drug Screening: Minimum 10-panel test including THC within 60 days of start (special rules for CA locations)

- Prime OIG Exclusion Check
- Prime Criminal Background Check (National, within 1 year; some states require before start)
- Prime SAM/EPLS Exclusion Search
- Prime Agency Attestation (uploaded by agency after credentialing)

- Annual Respirator Fit Test (must be completed prior to start if not done onsite)
- Positive PPD with Negative Chest X-Ray (optional)
- Name Verification Affidavit (optional, if name discrepancies exist)
Additional Notes
- Badge photo will be taken on the first day; no need to submit beforehand.
- Some immunization declinations are not accepted depending on location.
- Fit testing locations vary; coordination required if onsite testing is allowed.
- Drug screening requirements differ for California facilities due to state law.
