Description
Job Opportunity: RN CVICU - Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: December 22 (Non-negotiable)
Shift: 36 or 48 hours per week
Holiday Requirement: Must work 1 of 2 holidays
Required Credentials & Onboarding Requirements
Licenses & Professional Credentials
- Nursys Verification: Required within 30 days prior to start. Must disclose any disciplinary actions with completed FACIS Questionnaire.
- Current Nursing License: Primary source verification within 30 days prior to start.
- Driver's License/State ID: Must be current, match address on file, and show front/back.
- Resume/Profile: Required for submission.
- Two Professional References: Required prior to first assignment.
- Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and others. Must be completed within 30 days of first assignment and updated annually or after breaks in service.
- Education Verification: Must be completed within 90 days of start and updated if break in service > 90 days.
- Employment Verification: Past 7 years, updated annually or after breaks in service.
- Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
Certifications
- BLS (AHA): Required prior to onboarding.
- ACLS (AHA): Required prior to onboarding.
- NIH Certification: Group A updated yearly; Groups B-F every 2 years.
- Tdap: Valid within 10 years, no declinations accepted.
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics. Completed within a year of start, updated yearly. Minimum 80% score, max 2 attempts.
- Ascension EKG Exam: Must pass with at least 80%, max 2 attempts. Updated edition as of Dec 1, 2023.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
Personal Health & Immunizations
- MMR: Proof of two vaccines or positive IGG titer. No declinations accepted. If negative, two doses four weeks apart required.
- Varicella: Proof of two vaccines, serological immunity, or verified history. No declinations accepted. If negative, two doses four weeks apart required.
- Influenza Vaccine: Required for flu season (Oct 1 - Mar 31). Exemptions accepted with mask requirement.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Fit Test: Required for all travelers, preferred before start, can be done onsite. Annual update required.
- 2 Step PPD or IGRA: 1st PPD within 1 year, 2nd within 30 days of start or negative IGRA within 90 days. TB questionnaire updated annually.
- Positive PPD: Negative chest X-ray within 1 year and annual TB questionnaire required.
- Health Statement: Statement of good health or physical exam prior to first assignment, updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified substances.
Onboarding & Testing
- Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire training with attestation.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Color Vision Test: Completed prior to start, updated every 3 years.
- Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension Unit Specific Checklist: Optional for onboarding, uploaded by unit leader within a week.
