Job Opportunity: Therapy/Rehabilitation - Kenosha, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks
Shift: 5x8 hour day shifts
Start Date: December 15
Locals Accepted: Yes, at a lower rate
First Time Travelers: Not accepted
Requirements
- Licensure: State License required (pending license accepted)
- Certifications:
- BLS (Basic Life Support) - ARC or AHA acceptable; AHA Provider, Instructor, and RQI also accepted
- Background Check:
- 7-year background check required within 1 year of start, updated annually
- Includes SS Trace, County searches for last 7 years of locations lived/worked/schooled, Nationwide, Sex Offender checks
- References:
- 2 professional references verified within 2 years of start
- For inpatient RNs (MS, MS/TELE, IMC, Stepdown, ICU): 1 reference must be from most recent acute care assignment with performance comments
- Physical Exam: Required within 1 year of start
- Drug Testing: Negative 9-panel drug screen within 1 week of start (instant acceptable)
- Immunizations / Health Documents:
- Negative TB test (QGold) within 1 year of start (TSpot/PPD not accepted)
- Rubeola (Measles), Mumps, Rubella, Varicella: Positive titer or 2 vaccines required; low/equivocal titers require proof of 2 vaccines post titer
- Hepatitis B: Titer, series, or declination required
- Flu Seasonal Vaccination or approved medical/religious exemption required during flu season (Oct-Mar)
- Photo for Badge: Passport-style photo required (facing camera, no filters)
Additional Certifications (as applicable)
- ACLS (Advanced Cardiac Life Support) - ARC or AHA acceptable; not required for CNAs
- PALS (Pediatric Advanced Life Support) - ARC or AHA acceptable
- NRP (Neonatal Resuscitation Procedure) - as required
- NIH Stroke Certification - required for ICU, IMC/IMCU, inpatient, Med Surg, Step Down RN positions; must be renewed annually
- Tech Certification Verification - as applicable (refer to Allied Licensure report)
Competencies (as applicable)
- Medication Test/Pharmacology Competency Exam
- Color Blind Testing (required for dialysis/kidney center roles)
- Competency Exam relevant to profession/specialty
Facility-Specific Documents
- Compliance Checklist to be completed by agency representative once candidate is compliant
- Confidentiality Statement to be signed prior to start
- Flu Vaccine Religious or Medical Exemption Forms if applicable
- Dosimeter Request Form (required for Imaging Department only)
