Description

Job Opportunity: RN Medical/Surgical

Location: Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: December 15 (Non-negotiable)
Hours: 36 or 48 hours per week


Key Requirements & Credentials

Licenses & Certifications

  • Nursys Verification: Required within 30 days prior to start. Complete FACIS Questionnaire if disciplinary actions exist.
  • Current Nursing License: Primary source verification within 30 days prior to start.
  • BLS (AHA): Must be current and valid.
  • NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
  • Ascension Drivers License: Required for RN/LPN/PTs working on a compact license if placement is out-of-state. Address must match home state on compact license.
  • Driver’s License/State ID: Must be current, not expired, and match address on file.

Testing & Exams

  • Ascension Medical Surgical Exam: Must be completed prior to start; minimum 80% score; max 2 attempts.
  • Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly; minimum 80% score; max 2 attempts.
  • Ascension EKG Exam: New edition as of Dec 1, 2023; passing score of 80% required; max 2 attempts.
  • Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
  • Ascension Skills Checklist: Clinical skills checklist to be completed within one year of start and updated annually.

Health & Immunizations

  • MMR: Proof of two vaccines or positive IGG titer required; no declinations accepted.
  • Varicella: Proof of two vaccines, serological immunity, or verified history required; no declinations accepted.
  • Tdap: Valid within 10 years; no declinations accepted.
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted but mask required.
  • Hepatitis B: Documentation of vaccination series completion or signed declination required.
  • Fit Test: Required for all travelers; preferred before start, can be done onsite. Annual update required.
  • 2 Step PPD or IGRA: Required with specific timing; TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year.
  • Positive PPD: Negative chest X-ray and annual TB questionnaire required (optional but recommended).
  • Health Statement: Statement of good health or physical exam prior to first assignment; updated annually.
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start and updated annually; includes 12 specific drugs; retesting required if break in service > 90 days.

Onboarding & Compliance

  • Ascension OFAC Check: Required prior to onboarding.
  • Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or if break in service > 90 days.
  • Ascension EPLS: Must be run within 90 days of start; rerun if break in service > 90 days.
  • Ascension Onboarding Requirements Education Attestation: Completion of 6-hour pre-hire module with attestation.
  • Ascension Nursing Contract Worker Onboarding: Completed onsite.
  • Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
  • Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
  • Ascension Employment Verification: Third-party verification for past 7 years; updated annually or if break in service > 90 days.
  • Ascension Profile (Resume): Required for submission.
  • Two Professional References: Required prior to first assignment.
  • Ascension Badge Photo: Forward-facing headshot with white or blue background; no alterations.

Optional

  • Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
  • Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
  • Hiring Manager Evaluation: Post-assignment evaluation.
  • Ascension Name Change Document: Legal documentation of name change if applicable.

This position requires strict adherence to credentialing, health, and onboarding requirements prior to start and throughout the assignment.
