Description
Job Opportunity: RN Medical/Surgical
Location: Nashville, Tennessee
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Start Date: December 15 (Non-negotiable)
Shift Hours: 36 or 48 hours per week
Key Requirements and Credentials
Licenses & Certifications
- Nursys Verification: Must be completed within 30 days prior to start. Complete FACIS Questionnaire if any disciplinary actions.
- Current Nursing License: Primary source verification within 30 days prior to start. FACIS Questionnaire required if disciplinary actions exist.
- BLS (AHA): Required prior to onboarding.
- NIH Certification: Group A updated yearly; Groups B-F updated every 2 years.
- Driver’s License/State ID: Must be current, match address on file, and show front/back.
Testing & Exams (Required Before or At Onboarding)
- Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years.
- Ascension EKG Exam: Minimum 80% score, max 2 attempts, updated as per latest edition.
- Ascension Core Competencies: Includes HIPAA, Joint Commission NPSG, OSHA topics; minimum 80% score, max 2 attempts, updated yearly.
- Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation required.
- Ascension Skills Checklist: Clinical skills checklist, completed within one year of start and updated annually.
- Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually. Includes testing for 12 specified substances. Retesting required if break in service > 90 days.
Health & Immunizations
- MMR: Proof of two vaccines or positive IGG titer required; no declinations accepted.
- Varicella: Proof of two vaccines, serological immunity, or verified history required; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Fit Test: Required for all travelers; preferred before start, can be done onsite (except PAPR).
- 2 Step PPD or IGRA: Required with specific timing; TB questionnaire updated annually. Positive TB requires chest X-ray within 1 year.
- Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually.
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
Onboarding & Documentation
- Ascension Resume/Profile: Required for submission.
- Professional References: At least two professional references required prior to first assignment.
- Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations.
- Ascension Onboarding Requirements Education Attestation: Completion of pre-hire 6-hour module with attestation.
- Ascension Nursing Contract Worker Onboarding: Completed onsite.
- Ascension Nova StatStrip: Pre-hire modules required for non-allied positions.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates for extensions.
- Ascension Employment Verification: Verification of past 7 years employment; updated annually or after break in service > 90 days.
- Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; updated if break in service > 90 days.
- Ascension Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check; updated annually or after break in service > 90 days.
- Ascension EPLS: Must be run within 90 days of start; rerun if break in service > 90 days.
Optional
- Ascension Unit Specific Checklist: Optional for onboarding; uploaded by unit leader within a week.
- Positive PPD - Negative Chest X-Ray and Annual TB Questionnaire: Optional but recommended.
- Hiring Manager Evaluation: Post-assignment evaluation.
- Ascension Name Change Document: Legal documentation of name change if applicable.
