Job Opportunity: Physical Therapist (Rehabilitation Therapy)

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks

Job Description

Provide one-on-one therapy care for patients in their home setting.
Assess patients' conditions and develop individualized plans of care to help them reach their goals and maximize mobility and independence.
Collaborate with interdisciplinary team and colleagues to ensure patients' needs are met.
Perform standard therapy measurement techniques, including range of motion (ROM), strength, and balance tests.
Utilize a variety of therapeutic modalities to maximize care provided to clients.

Requirements

Vermont Physical Therapist licensure.
Home Health experience and OASIS experience required (candidate will case manage).
Experience with McKesson/Netsmart EMR strongly preferred.
