Physical Therapist (PT) - Rehabilitation Therapy
Location: Petaluma, California
Employment Type: Contract
Contract Length: 13 weeks
Position Overview
As a Physical Therapist, you will assess and treat patients to help them regain physical function and improve mobility. You will work within a collaborative healthcare team to design and implement individualized treatment plans that promote independence and enhance residents' quality of life.
Key Responsibilities
- Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
- Provide physical therapy interventions to improve strength, mobility, and functional abilities.
- Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications.
- Collaborate with interdisciplinary team members (occupational therapists, nurses, physicians) to coordinate care and optimize outcomes.
- Monitor patient progress and adjust treatment plans as needed.
- Maintain accurate and timely documentation of assessments, progress notes, and treatment plans.
- Ensure care plans comply with regulatory requirements and best practices in physical therapy.
Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
- Valid Physical Therapy license in California.
- Experience in a skilled nursing facility or similar healthcare setting preferred.
- Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively.
- Compassionate, patient-focused approach to care.
