Registered Nurse Travel/Contract Position
Location: Charleston, West Virginia
Contract Length: 13 weeks
Job Overview
- Position is a travel/contract Registered Nurse role.
- Facility is part of a hospital system in Charleston, WV.
Required Documents & Background Checks
- Valid Driver’s License (required).
- Social Security Card (legible; can clear with pending card if I9 and E-Verify are complete).
- Residency Declaration Form (if working on a compact license, driver's license must match compact state).
- 7-year Criminal Background Check within 30 days of start (includes SSN Trace, Counties, Sex Offender Search, National Criminal, and International searches if applicable).
- EPLS/GSA/SAM, HHS/OIG, and National Sex Offender Registry checks must be completed independently within 30 days of start and updated annually.
Certifications
- BLS (Basic Life Support): AHA only, must be current before start.
- ACLS (Advanced Cardiac Life Support): AHA only, required except for certain units (3 South, 3 West, 4 West, 4 South, 5 West).
- PALS (Pediatric Advanced Life Support): Required for ER, PACU, PAC, PICU, Children’s Cancer Center; not required for ER Clinical Decisions Unit.
- NRP (Neonatal Resuscitation Procedure): May be required for L&D, NICU, Mother/Baby.
- CHEMO (Chemotherapy Certification): Required depending on unit; highly preferred but not required for 5 South.
- TNCC (Trauma Nursing Core Course): Required for ER generally, but not required for ER at this facility.
- CPI (Crisis Prevention Institute): Required for ER units, Surgical & Trauma ICU, Trauma Stepdown, and Behavioral Health. Must have CPI Blue Card (online courses not accepted).
- Pass/Fail Testing (EKG, Medication Calculation, Blood Administration): Required for nursing candidates only; two attempts allowed per exam.
- State License Verification: Primary source verification within 30 days of start.
- NURSYS Verification: Must be run within 30 days of start.
- Additional certifications may be required based on unit (e.g., ACT and AVOX for Cath Lab RN).
Competencies
- Medication Test/Pharmacology Competency Exam required for all nursing staff.
- Color Blind Testing required prior to start.
- Unit-specific competency exams required (e.g., MS/Tele exam for Clinical Decisions unit).
Health & Immunization Requirements
- Physical exam within 1 year of start, signed by a licensed provider, confirming good health and no physical limitations.
- Annual Fit Mask testing (3M N95 1860 Small or Regular and Aura 9205 accepted). PAPR accommodations available if needed.
- Tdap vaccination required (given after age 11). Medical and religious exemptions allowed with approval.
- Negative TB test or chest X-ray within 3 months of start; annual TB questionnaire required for positive TB responders.
- MMR (Measles, Mumps, Rubella): 2 vaccines or positive titers required; boosters required if titers are negative or equivocal.
- Hepatitis B: Documentation of vaccination series or positive titer required; declination allowed if series completed and titer negative.
- Varicella: 2 vaccines or positive titers required; boosters required if titers negative or equivocal.
- Drug screening within 30 days of start, including a comprehensive panel; no placeholders or pending results accepted.
- Seasonal Flu vaccination or approved exemption required annually; strict compliance deadlines apply.
- COVID-19 vaccination is not mandatory; proof of vaccination or signed declination accepted.
Facility-Specific Requirements
- Orientation is two days (Thursday and Friday), including Cerner Infusion Management training.
- Review of Blood Administration Policy & Reference Chart required prior to start.
- Dress Code Policy must be reviewed, initialed, and returned.
- RN/LPN Math Calculation Packet and EKG Review packets must be reviewed prior to start (no tests at orientation).
- Inservice Training modules must be completed approximately 1 week before start (approx. 4 hours). Includes pass/fail testing for certain specialties.
- Time and Attendance policy must be reviewed and acknowledged.
- Medical Release Form must be completed and returned.
- Vaccination exemption request forms (medical or religious) must be completed if applicable and approved before start.
- Application Request and Delegate Forms must be completed as part of credentialing.
- WV State Liability and Time Gap Forms required at credentialing start.
References & Skills
- One reference required from a manager, supervisor, or charge nurse from a facility worked within 1 year of start.
- Annual skills checklist required for specialty; Clinical Decisions unit requires MS/Tele skills checklist.
