Job Opportunity: RN Neuro (Long-Term Assignment)

Job Description

• On Call: As assigned
• Minimum call requirements: 10-12 call shifts per month
• Holiday Rotation: Expected to participate in on-call team during holidays
• Mostly day shifts
• Shift length: Varying 8-hour shifts depending on department needs

Required Certifications & Licenses

• BLS (Basic Life Support): American Heart Association certification required for all direct patient care personnel
• ACLS (Advanced Cardiovascular Life Support): Required for RNs in critical care areas
• NIHSS (National Institutes of Health Stroke Scale): Required
• License/Certificate Verification: Must be dated within 30 days of start, including state verification (NURSYS for RNs)
• TNCC (Trauma Nursing Core Course): Optional

Health & Immunization Requirements

• Fit Test
• Trinity 10 Panel Drug Screen: Completed within 30 days of start, includes testing for multiple substances (NO THC)
• COVID-19 Vaccine: Proof of booster by Sept 2025, or declination/exemption (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Influenza Vaccine: Proof of vaccine, declination, or exemption for flu season 10/1/2024 - 4/30/2025
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing with chest x-ray if positive (within last year for some states, within 5 years for others)
• Varicella: Proof of 1 vaccination or IgG titer lab report
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work
• Tdap: Current, non-expired vaccine required; declinations accepted if no pediatric interaction

Professional & Compliance Requirements

• Trinity Code of Conduct Form: Signed and on file
• Trinity I9: Completed by agency
• Confidentiality & Security Agreement: Signed form required
• Integrity and Audit Services Form: Signed prior to start
• Non Employee Upload Form: Completed and uploaded
• Specialty Specific Competency Testing: Specialty exam within past year
• Conflict of Interest Disclosure Form: Signed and on file
• Criminal Background Check: Federal, nationwide, county checks for last 7 years, including SSN verification, dated within 30 days of start
• Education Verification: Highest academic degree or diploma verified
• EPLS (Excluded Parties List System) Check: Dated within 30 days of start
• OIG (Office of Inspector General) Check: Dated within 30 days of start
• Joint Commission CORE Exam: Within past year
• NSO (National Sex Offender) Check: Dated within 30 days of start, including all aliases
• System Access Form: Completed and uploaded one week before start
• Skills Checklist: Provided by agency for clinical contractors
• Child and Adult Dependent Abuse Training: Required for Iowa assignments
• Iowa Sing Report: Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required within 30 days prior to start
• OFAC (Office of Foreign Assets Control) Check: Completed within 30 days of start
