Job Opportunity: Occupational Therapist (OT) - Travel/Contract
Location: Saint Cloud, Minnesota
Specialty: Therapy/Rehabilitation

Contract Length: 13 weeks
Job Details
- Work schedule: 5 days a week, 8 hours per day (0730-1600)
- Every 4th weekend required to work
- Must have previous NICU experience
- Must have an active license or have already applied (estimated time to approval: 6 weeks)
- Minimum 1 year of experience required
- EPIC experience preferred
Required Certifications & Competencies
- Licensure: Must have or be in process of obtaining a valid OT license
- Background Checks:
- Criminal background check within 1 year, including 7-year search, SS trace, counties, sex offender, nationwide, education verification (not required for CNAs), and international searches if applicable
- EPLS/GSA/SAM and HHS/OIG excluded provider checks required
- Fingerprinting via Minnesota DHS Net Study required (candidates cannot start with fingerprints pending unless approved with a "more time needed" letter)
- Education Verification: Third-party verification required (diploma acceptable with proof of verification attempts)
- Competency Exams: Required as applicable, must meet minimum score, updated annually
- Medication Test/Pharmacology Competency Exam: Required for RNs (not required for NICU RNs)
- Basic Life Support (BLS): AHA BLS certification required
- Advanced Cardiac Life Support (ACLS): Required for some roles (not specifically stated for OT)
- Neonatal Resuscitation Program (NRP): Required for NICU roles
- Other Certifications: May include Pediatric Advanced Life Support (PALS), Trauma Nursing Core Course (TNCC), Electronic Fetal Monitoring (EFM) as applicable
Health & Immunization Requirements
- Physical exam within 1 year of start
- Fit testing for N95 masks (Halyard Fluid Shield or 3M 1860) required
- Tetanus/Diphtheria/Tdap vaccination required (no religious declinations accepted)
- Negative TB test or chest x-ray per Minnesota state guidelines (2-step PPD or equivalent)
- MMR (Measles, Mumps, Rubella) proof of immunity or vaccination required (time sensitive)
- Hepatitis B vaccination, positive titer, or declination accepted
- Varicella immunity or vaccination required
- Seasonal flu vaccination or declination required annually (mask required if declined)
- COVID-19 vaccination preferred; medical/religious exemptions accepted
- COVID-19 booster required only for long-term care candidates with specific testing and masking protocols if not boosted
- Drug testing: 9-panel test within 1 year of start (instant acceptable)
Facility Specific Requirements
- Photo for badge: Grey background, no selfies or professional photos unless headshot, JPG format
- Dress code: Review required prior to start; RRTs wear dove gray scrubs
- Education modules: Approximately 16 hours, completed during orientation (except some locations)
- Confidentiality agreement must be signed prior to start
- Badge must be returned on last day of assignment
Additional Notes
- Two references from recent employers within 2 years required (non-clinical candidates)
- Skills checklist required for all clinical candidates
- Work history verification required for non-clinical candidates (3 years)
- Candidates must complete Minnesota DHS Net Study Fingerprint Questionnaire immediately after signing contract
- Candidates cannot start without fingerprint clearance or approved exemption
- Fingerprinting must be completed within 2 weeks of registration
- Candidates may need to travel to a location offering livescan fingerprinting if not available locally
