Location: Waterloo, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Work Schedule

• Every Other Week (EOW) / Every Other Weekend (EOH)

Required Certifications & Licenses

• BLS (American Heart Association certification required for all direct patient care personnel)
• ACLS (Required for RNs in CCU, ED, PACU, MICU, SICU)
• Valid nursing license with verification dated within 30 days of start
• TNCC (optional)
• NIHSS

Health & Safety Requirements

• Fit Test
• Trinity 10 Panel Drug Screen (10-panel drug screen completed within 30 days of start; excludes THC)
• COVID-19 Vaccine (proof of booster by Sept 2025, declination or exemption accepted except in CA or IL)
• Influenza Vaccine (proof or declination for 10/1/2024 - 4/30/2025 period; declination not accepted in CA or IL)
• MMR (proof of 2 vaccinations or IgG titer)
• Hepatitis B vaccination series (proof of 1st vaccination prior to start; titers not accepted)
• Varicella (proof of 1 vaccination or IgG titer)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) with chest x-ray if positive TB as per regional requirements
• Physical/Health Statement (exam within last year confirming fitness to work)
• Tdap vaccine (current, non-expired; declinations accepted if no pediatric interaction)
• Trinity Color Vision test

Professional & Compliance Requirements

• Trinity Code of Conduct Form (signed)
• Trinity I9 form (completed by agency)
• Confidentiality and Information System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (completed and uploaded)
• Specialty Specific Competency Testing (copy of specialty exam within last year)
• Conflict of Interest Disclosure Form (signed)
• Criminal Background Check (Federal, Nationwide, County for last 7 years; SSN verification; dated within 30 days of start)
• Education Verification (highest academic degree or diploma verified)
• EPLS (Excluded Parties List System) check dated within 30 days
• GSA/OIG (Office of Inspector General) exclusion check dated within 30 days
• Joint Commission CORE Exam (within last year)
• NSO (National Sex Offender) check dated within 30 days, including aliases and locations
• System Access Form (completed and uploaded one week before start)
• Skills Checklist (provided by agency for clinical contractors)
• Child and Adult Dependent Abuse Training (required for Iowa)
• Iowa Sing Report (Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start)
• OFAC (Office of Foreign Assets Control) sanctions check within 30 days

Additional Notes

• All required documents and certifications must be uploaded to the credentialing system prior to onboarding.
• Specialty competency exam must be current within one year.
• Drug screen must be completed and resulted before start with Medical Review Officer (MRO) signature.
• License verification must be a scanned online verification page with a machine-generated date within 30 days of start.
• Vaccine declinations are not accepted in California or Illinois for COVID-19 and Influenza vaccines.
• Physical exam documentation must confirm candidate is fit to work and be dated within the last year.

This position is ideal for experienced critical care RNs who meet the above credentialing and health requirements and are available for a 13-week assignment in Waterloo, Iowa.
