Job Opportunity: Physical Therapist (Travel/Contract)
Location: Stockton, California

Contract Length: 13 weeks
Job Description
- Field position covering Central Valley/Stockton territory
- Home Health setting
- Work schedule options: Tuesday-Saturday or Wednesday-Sunday
- Local candidates are welcome
- Home Health experience preferred
- California license preferred; candidates with pending CA license who have applied may be considered
- OASIS experience required; candidates without OASIS experience may be considered if they have strong Home Health experience
- Guaranteed hours based on completed visits: 25 visits = 40 hours per week
- COVID exemptions permitted
- Mileage reimbursed at IRS standard rates
Required Credentials & Documentation
Licenses & Certifications
- California Physical Therapy License (preferred or pending with application submitted)
- OASIS experience required
- ACLS (AHA Instructor or Provider) – as applicable
- BLS (AHA Instructor or Provider) – required for all
- PALS, NRP, EFM – as applicable
- National Tech Certification Verification – as applicable
- Primary source verification of state license
- NURSYS verification (for RN, LPN/LVN, AP if applicable)
Background Checks
- Standard background check (7-year search including SS trace, county, nationwide, sex offender)
- EPLS/GSA/SAM check (annual)
- HHS/OIG check (annual)
Competency Exams
- Medication Test/Pharmacology Competency Exam (as applicable, minimum 80% or Angoff score)
- General Competency Exam (minimum 80% or Angoff score)
Facility Specific Documents
- Passport-style photo (shoulders up, front facing, good lighting, plain background)
- Personal or auto insurance (must be current and updated during assignment)
- Auto registration (must be current and updated during assignment)
- Training content acknowledgment
- Hand hygiene policy acknowledgment
Health Requirements
- Physical exam within 12 months of start
- TDAP vaccine within past 10 years (no declinations accepted)
- Negative TB test or chest X-ray within 1 year (with additional requirements if past positive)
- Proof of immunity or vaccination for Rubeola (Measles), Mumps, Rubella (MMR) for candidates born in 1957 or later (no declinations accepted)
- Hepatitis B titer or vaccine series (declination accepted if within 1 year of start)
- Varicella immunity or vaccination (no declinations accepted)
- Negative 9-panel drug screen within 1 year (instant drug screen acceptable)
- Seasonal flu vaccination/documentation (declinations accepted)
- COVID-19 vaccination or medical/religious exemption required prior to start
- COVID-19 booster required
History & References
- Two professional references required
- Annual skills checklist required
This position is ideal for Physical Therapists with home health experience and OASIS knowledge who are licensed or in the process of obtaining a California license and are comfortable working in a travel/contract role within the Central Valley region.
