Physical Therapist Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

• Provide physical therapy services to patients in their homes with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
• Patient age range: birth to over 100 years.
• Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
• Maintain thorough and timely electronic documentation and interdisciplinary communication per agency policies and standards.
• Weekend rotations occur once every 6 weeks.
• Ability to travel to assigned cases is required.

Requirements

• Must hold a valid Physical Therapist professional license in Massachusetts.
• Minimum 1 year of home care experience required.

Onboarding & Credentialing Requirements

• Professional Licenses & Verifications:

  • Massachusetts Physical Therapist license (no conditional licenses accepted).
  • Primary source verification of license within 30 days of start and updated upon expiration.
  • Resume including past 7 years of work experience with city and state.
  • Two recent references (minimum one supervisor or above in skillset).
  • Skills checklist required for submission.
  • Criminal background check, including Massachusetts CORI, national sex offender check, exclusion checks, and SSN trace.
  • Motor Vehicle Record (MVR) check within 45 days of start (required for all home care positions).
  • Education verification of highest level achieved.
  • Confirmation of assignment form upon acceptance and for extensions.

• Health & Immunization Requirements:

  • N95 Fit Test within 365 days of start and updated annually (no religious exemptions).
  • Respirator Medical Clearance attestation (part of fit test).
  • TB test documentation within 3 months of start (skin test or blood test) and updated if >6 month break.
  • Health statement signed by MD/DO/NP/PA within 365 days of start and updated if >6 month break.
  • Influenza vaccine required during flu season (Sept 15 - April 30), no exemptions.
  • MMR vaccination or proof of immunity.
  • Hepatitis B vaccination or immunity proof strongly preferred (optional).
  • Tdap vaccine within 10 years preferred (optional).
  • If positive TB test, documentation of negative chest x-ray required (optional).
  • TB symptom survey if positive PPD (optional).

• Additional Notes:

  • MGB badge photo required for onboarding (color photo, no selfies, plain background, no eyeglasses).
  • All background and credential checks must be current and updated as required.

This position requires a licensed Physical Therapist with home care experience, strong collaboration skills, and compliance with comprehensive credentialing and health requirements to provide in-home physical therapy services in Massachusetts.
