Job Description
- Position: X-ray Technician
- Schedule: Monday to Thursday, 9:00 PM to 6:30 AM
Required Credentials and Compliance
Certifications & Licenses
- BLS (Basic Life Support): Must be AHA certified.
- Current License/Certificate Verification: Must be verified within 30 days prior to assignment start.
- Driver's License/State ID: Must be valid, not expired, and match address requirements.
Professional Requirements
- Ascension Core Competencies: HIPAA, Joint Commission NPSG, OSHA topics; completed within a year of start and updated yearly with a minimum 80% score (max 2 attempts).
- Ascension Medical Imaging Contract Worker Onboarding Education Attestation: 6 hours pre-hire education with signed attestation.
- Ascension Profile: Resume or professional profile required.
- Ascension 2 Professional References: At least two professional references prior to first assignment.
- Ascension Criminal Background Check: Includes Social Security Trace, Employment Verification, OIG Search, National Sex Offender check; completed within 30 days of first assignment and updated annually or after breaks in service.
- Ascension EPLS (Excluded Parties List System): Run within 90 days of start; rerun if break in service > 90 days.
- Ascension Skills Checklist: Clinical skills checklist completed within one year of start and updated annually.
- Ascension Education Verification: Verification of education relevant to clinical work within 90 days of start; rerun if break in service > 90 days.
- Ascension Confirmation of Assignment: Provided upon initial assignment and updates with extensions.
- Ascension Employment Verification: Third-party employment verification for past 7 years; updated annually or after breaks in service.
Personal Health History & Immunizations
- MMR (Measles, Mumps, Rubella): Documented proof of two vaccines or positive IGG titer; no declinations accepted.
- Fit Test: Required for all travelers; preferred prior to start, can be completed onsite; updated annually.
- Hepatitis B: Documentation of vaccination series completion or signed declination.
- Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); exemptions accepted with mask requirement.
- Positive PPD / Negative Chest X-Ray and Annual TB Questionnaire: Must provide positive IGRA or PPD results, chest X-ray within a year showing no infection, and completed TB questionnaire; updated annually.
- Health Statement: Statement of good health or completed physical exam prior to first assignment; updated annually.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required within specified timeframes prior to start; TB questionnaire updated annually.
- Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider; no declinations accepted.
- Tdap: Valid within 10 years; no declinations accepted.
- Ascension 12 Panel Drug Screen: Completed within 30 days prior to start; includes testing for 12 specified drugs; must include MRO review for prescriptions or negative-dilute results.
- Ascension Influenza Vaccine Exemption/Declination: If exempted, mask wearing required during flu season.
Additional Notes
- Medical Imaging Contract Worker Form is completed onsite by the facility.
- Forward-facing badge photo with white or blue background required (no alterations or filters).
- Legal documentation of name change may be required if applicable.
- Post-assignment evaluation may be conducted.
This position requires strict adherence to credentialing, immunization, and compliance standards to ensure patient and staff safety.
