Travel RN Medical/Surgical Position - Indiana (Evansville)

Unit & Patient Details

  • Specialty: RN Medical/Surgical
  • Unit: Renal/Diabetic Unit
  • Patient Ratio: 1:6-8
  • Beds: 23

Contract Length

  • 13 weeks

Required Certifications & Licenses

  • BLS (American Heart Association)
  • Current nursing license with Nursys verification within 30 days prior to start
  • License/Certificate primary source verification within 30 days prior to start
  • Driver's License/State ID (valid, must match address requirements)

Required Exams & Testing (Must be completed prior to start and updated as specified)

  • Ascension Pharmacology Exam (minimum 80%, updated every 3 years)
  • Ascension Medical Surgical Exam (minimum 80%, updated every 3 years)
  • Ascension Color Vision Test (updated every 3 years)
  • Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, updated yearly)
  • Ascension EKG Exam (minimum 80%, updated as new editions released)
  • Fit Test (annual, preferred prior to start; specific mask types required)
  • Ascension 12 Panel Drug Screen (within 30 days prior to start, updated annually)
  • Ascension Skills Checklist (clinical specialty, completed within one year of start, updated annually)

Health & Immunization Requirements

  • MMR: Proof of two vaccines or positive IGG titer (no declinations)
  • Hepatitis B: Vaccination series completion or signed declination
  • Influenza Vaccine: Required during flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
  • Positive PPD with Negative Chest X-Ray and annual TB Questionnaire (or equivalent IGRA test)
  • 2 Step PPD OR IGRA (Quantiferon/Tspot) within 30 days prior to start
  • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations)
  • Tdap: Valid within 10 years (no declinations)
  • Health Statement or physical exam prior to first assignment, updated annually

Onboarding & Professional Requirements

  • Ascension Onboarding Requirements Education Attestation
  • Ascension Nursing Contract Worker Onboarding (completed onsite)
  • Ascension Badge Photo (professional headshot with white or blue background)
  • Ascension Profile (resume or professional profile)
  • At least two professional references prior to first assignment
  • Criminal Background Check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender Registry (completed within 30 days prior to start, updated annually)
  • Employment Verification (third-party, past 7 years, updated annually)
  • Education Verification (within 90 days prior to start)
  • Ascension Confirmation of Assignment (agency letterhead documentation of assignment dates)

  • Ascension Exit Survey (post-assignment)
  • Ascension Unit Specific Checklist (optional for onboarding)
  • Hiring Manager Evaluation (post-assignment)
  • Legal documentation of name change (if applicable)
