Job Description

• Position: Registered Nurse (RN) in Labor and Delivery (L&D)
• Shift: Night shift
• Experience: Minimum 5 years of relevant experience required

Required Certifications and Credentials

• BLS (American Heart Association certification): Required for all direct patient care personnel. Must provide scanned copy of both front and back of card with expiration date.
• NRP (Neonatal Resuscitation Program): Required for RNs in Birth Center. Scanned copy with expiration date needed.
• License/Certificate Verification: Must be dated within 30 days of start. Online verification required (state verification preferred).
• Fit Test: Required prior to onboarding.
• Trinity 10 Panel Drug Screen: Completed within 30 days of start, preferred within 14 days. Must include specific substances (no THC).
• COVID-19 Vaccine: Proof of vaccine, booster, declination, or exemption required (declination not accepted in CA or IL).
• MMR Vaccine: Proof of 2 vaccinations or IgG titer lab report required.
• Hepatitis B: Proof of vaccination series required (titers not accepted).
• Influenza Vaccine: Proof of vaccine, declination, or exemption required for current flu season (declination not accepted in CA or IL).
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; positive TB requires chest x-ray within specified timeframes.
• Varicella: Proof of 1 vaccination or IgG titer lab report required.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.

• Trinity Specialty Specific Competency Testing: Specialty exam must be completed within a year.

• Trinity Conflict of Interest Disclosure Form
• Trinity Criminal Background Check: Federal, nationwide, county criminal check from last 7 years, including SSN verification and employment verifications. Must be dated within 30 days of start.
• Trinity Education Verification: Verification of highest academic education level required.
• Trinity EPLS (Excluded Parties List System): Must be dated within past 30 days.
• Trinity GSA/OIG Exclusion Check: Must be dated within past 30 days.
• Trinity Joint Commission Testing (CORE Exam): Must be within a year.
• Trinity NSO (National Sex Offender) Check: Must be dated within 30 days of start.
• Trinity System Access Form: Must be completed and uploaded one week before start.
• Trinity Skills Checklist: Required for clinical contractors; agency will provide checklist.

Additional Notes

• All required documents and certifications must be uploaded to the designated system prior to onboarding.
• Drug screening excludes THC.
• Declinations for vaccines are not accepted in California or Illinois.
• Specialty competency testing and skills checklist must be current (within the last year).
