Travel/Contract Occupational Therapist (Adult OT)

Location: Sarasota, Florida
Specialty: Therapy/Rehabilitation
Contract Length: 20 weeks


Job Description

  • Provide skilled home health occupational therapy services focusing on adult patients.
  • Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson’s, failure to thrive, respiratory patients, neuro, trachs, trauma, amputees (AKA, BKA), and ADLs.
  • Home skilled visits: 6-8 visits per day, minimum 6 points per day (may increase at manager’s discretion).
  • Must be an independent practitioner with at least 1 year of home health experience (2 years preferred).
  • Must have experience with OASIS documentation (training preferred).
  • Evaluation tools and treatment skills include:
    • Activities of daily living (ADLs)
    • Cardiac rehab
    • Edema massage
    • Feeding techniques
    • Splinting
    • TENS and Ultrasound
    • Orthopedics and Prosthetics/Orthotics
    • Standardized assessments
    • Joint mobilization
    • Home Exercise Program development
    • Lifting and transfer techniques
    • Patient and family education
    • Safety and injury prevention
    • Telephone communication
  • Documentation and billing using BayCare Forms, BayCare Scheduling, and Ace Timesheet system.
  • Visits and hours are self-scheduled based on HomeCare office needs.
  • Must have a valid driver’s license and reliable vehicle for home visits.

Required Certifications and Licenses

  • Current state OT license.
  • Basic Life Support (BLS) certification (AHA or RQI accepted).
  • COVID-19 vaccination or approved exemption on file.
  • Flu vaccination or declination with mandatory mask wearing at all times while working.

  • Portable printers

Dress Code

  • Any solid color scrubs.

Compliance and Documentation Requirements

  • Valid driver’s license must remain active throughout assignment.
  • Background checks including 7-year history, education verification, and state/federal screenings required.
  • Fingerprinting per Florida AHCA Level II requirements (must be cleared prior to start).
  • Physical exam within 6 months of start date.
  • Negative TB test or chest X-ray within 10 weeks of start (annual TB questionnaire required).
  • Immunizations or titers for Measles, Mumps, Rubella, Varicella, Hepatitis B.
  • Drug screening (standard 9-panel) within 30 days of start.
  • Annual flu and COVID-19 vaccination documentation or approved exemptions.
  • Respirator medical clearance if applicable.
  • Completion of BayCare Online Learning Center modules prior to start (4-8 hours).
  • Professional photo for badge required prior to start.
  • Personal auto insurance required and must remain current during assignment.
  • References from last 2 years and employment application must be submitted.
  • Skills checklist required.

Additional Notes

  • Candidates must be able to work independently in a home health setting.
  • Must be comfortable managing a varied patient population with complex medical and orthopedic needs.
  • Self-scheduling flexibility based on office needs.
  • Compliance with BayCare Health System policies and procedures is mandatory.
