Registered Nurse (RN) - Home Health Care
Location: Salisbury, Maryland
Employment Type: Travel/Contract
Contract Length: 13 weeks (approximately 3 months)

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patient load: 5-8 patients per day.
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Must be willing to travel up to 75 miles from the home clinic; average travel about 200 miles per week.
- Documentation and charting required using HomeCare HomeBase (HCHB) and OASIS systems.
- Schedule: Monday to Friday, 8 AM to 5 PM. No weekend rotation or on-call required.
- Territory: Wicomico/Worcester County, Maryland.
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (first-timers not accepted).
- Current Registered Nurse license in hand (no pending licenses accepted).
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months required; if not, retraining will be required.
- RNs must be proficient in OASIS documentation.
- Must have reliable transportation and valid driver's license.
Productivity Expectations
- Weekly productivity target: 30 points (points assigned based on types of visits and activities).
- Assigned 5-10 patients per day, equating to roughly 6-8 points daily.
Dress Code
- Scrubs required.
Travel and Mileage
- Must be willing to travel up to 75 miles from the home clinic.
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
- Local candidates accepted at the same rate.
Required Documentation and Compliance
- Valid Driver’s License/State ID.
- Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific checks as applicable.
- Fingerprinting as required by state regulations.
- Education verification for nursing degree.
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- 10-panel drug screen.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or documented declination.
- Completion of facility-specific compliance documents including confidentiality forms, HIPAA and privacy policies, code of conduct, compliance training, and home health scenarios.
- References: Two professional references required.
- Work history verification for the last 7 years or two most recent employers.
Additional Notes
- Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 Monday-Friday.
- Weekend and on-call requirements vary by location and profession; for this assignment, none are required.
- Open position due to RN maternity leave coverage.
This position is ideal for experienced Home Health RNs who are comfortable with travel, electronic medical records (HCHB and OASIS), and managing a moderate patient load in a home care setting.
