Job Details

• Work Schedule: Monday to Friday, with every 6th weekend required
• Patient Load: 5-20 visits per day on average
• Experience Required: Minimum 1 year of Home Health experience
• First-Time Travelers: Accepted
• Certifications Required: Basic Life Support (BLS)
• On-Call: Not required
• Floating: No floating outside scope of practice
• Local Candidates: Accepted (within 40-80 miles radius)

• Guaranteed Hours: 3 shifts guaranteed per 13 weeks (cancelled shifts must be full shift length)
• Return to Office (RTO) Restrictions: No more than 7 days off-site

Special Requirements

• Must have prior Home Health experience
• Government-issued driver's license required (for Home Health positions)
• Mileage reimbursement provided

Compliance & Documentation Requirements

Background Checks & Verifications

• Criminal background check within 6 months (7-year search including statewide and nationwide checks)
• EPLS/GSA/SAM and HHS/OIG quarterly checks
• Education verification of highest relevant degree (no pending at start)
• State-specific searches and registries as applicable (Missouri and Illinois)
• Fingerprinting required for Illinois long-term care or rehab assignments (not for IL Home Care)

Certifications

• BLS (Basic Life Support) - AHA only
• ACLS, PALS, NRP, TNCC, EFM, CPI/MAB as applicable by unit (check unit-specific requirements)
• State License verification (primary source verification required)
• IV Certification required for LPN/LVNs in Missouri
• Other certifications may be required depending on unit (e.g., NIH Stroke, MANDT for Behavioral Health)

Health Requirements

• Physical exam within 1 year of start
• Fit mask test (N95) annually, with specific procedures for failed tests
• Immunizations and titers:

  • Tdap (or Td booster) every 10 years
  • Negative TB test or chest x-ray (2 negative PPDs or T-Spot/Q-Gold within 1 year)
  • Measles, Mumps, Rubella, Varicella immunity or vaccination/declination
  • Hepatitis B declination form required (vaccination not mandatory)
• Drug testing within 1 year, including specific drug panels
• Seasonal flu vaccination or approved exemption required during flu season (Sept-April)

Facility-Specific Documents

• HIPAA Acknowledgement (annual)
• Latex Allergy Questionnaire (annual)
• Confidentiality Form (annual)
• Regulatory Compliance Education (annual, must pass with 80% or higher)
• TB Questionnaire (annual)
• Hand Hygiene, Fingernail, Dress Code Policies (must review and acknowledge)
• Badge photo and facility badge required
