Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Nursing (Clinical - Nursing)
Contract Length: 13 weeks

Job Description

Duties & Responsibilities:

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop and implement the plan of care for patients and families.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods for patients and families; evaluate and revise teaching plans as needed.
• Manage case management responsibilities including prioritizing care needs, revising care plans, and ensuring compliance with hospice regulations and insurance guidelines.
• Communicate progress toward goals with the interdisciplinary team (IDT) and participate in IDT meetings every 2 weeks.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including routine medication reconciliation and maintaining current medication orders.
• Participate in monthly staff meetings, performance improvement (PI) activities, utilization review (UR) meetings, and other committees as requested.
• Provide orientation and mentorship to new employees; cross-train and assist with agency growth initiatives.

Qualifications & Skills

• 2-3 years of experience as an RN in a home hospice environment.
• Current New Jersey RN license.
• Valid New Jersey driver’s license.
• BLS certification required.
• Experience using an Electronic Medical Record (EMR) system.
• Excellent critical thinking and communication skills.
