Physical Therapist Outpatient - Oak Bluffs, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Schedule: Monday to Friday, 9:00 AM - 5:00 PM
Contract Length: 13 weeks
Job Description
- Outpatient Physical Therapist position.
- Shared housing available (no guests or pets allowed).
- Must have own transportation.
- Current flu vaccine required.
Required Credentials and Compliance
Professional Credentials
- Resume: Include past 7 years of work experience with city and state details. (Required for submission)
- References: Two recent references, at least one supervisor or above in the relevant skillset. (Required for submission)
- Skills Checklist: Must be completed and submitted. (Required for submission)
- Criminal Background Check: Within 45 days prior to start and updated if >6 month break. Includes county and international searches if applicable.
- Massachusetts CORI Check: Completed within 30 days prior to start and updated if >6 month break.
- Education Verification: Highest level of education verified through background check.
- SSN Trace: Must be run with criminal check, covering last 7 years of addresses/aliases.
- National Sex Offender Check: Completed within 30 days of start and updated annually.
- Exclusion Checks: Includes OIG, GSA, MassHealth, and NH Medicaid lists; updated within 30 days of start and annually.
- Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
Licenses and Certifications
- License/Certificate Verification: Primary source verification of professional license/certification within 30 days of start. MA Conditional Licenses not accepted.
- BLS Certification: AHA, Red Cross, CHSF, or Military accepted. Preferred AHA for certain hospitals.
Personal Health History and Vaccinations
- Flu Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
- MMR: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
- TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start, updated if >6 month break.
- Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break.
- Respirator Medical Clearance Attestation: Required if fit test is needed for the role.
Optional but Preferred
- Hepatitis B: Strongly preferred; record of 3 vaccines or proof of immunity or declination.
- Tdap Vaccine: Preferred if within last 10 years.
- Positive PPD - Negative Chest X-Ray: Required if positive TB test.
Additional Notes
- Must comply with all credentialing and onboarding requirements prior to start.
- Shared housing requires signing a housing agreement if utilized.
- No guests or pets allowed in shared housing.
- Must have reliable transportation to work site.
